Symptomtext

Pt is GCS 15 and A&Ox4. Skin is P/W/D to pt ethnicity. Neuro intact and PERRLA. Respirations are regular/equal and non-labored. pt states he's had left sided facial droop for 2 days and a sore throat for 5 days. pt denies any other symptoms at this time. pt appears to have bells palsy. Dr. was contacted and she ruled out a stroke. Symptoms started 1 week after the completion of vaccine.

Symptomtext

Patient fainted, extremities were shaking, chills, weakness, dizziness. Paramedics took patient out on stretcher & at hospital did EKG, gave fluids, monitored vitals. All normal, but patient was dehydrated. Sent home and recovered.

Patient recently in hospital from 07/17 until 7/21. During that time she was here for headache/nausea/vomiting and possible mastoiditis. Note: SARS COV2 PCR negative at prior visit 7/18/21 7/25/21: She re-presented to hospital 7/25/21 for continued nausea and vomiting. ED vital signs revealed tachycardia but otherwise unremarkable and was found COVID Positive 7/25/21, prior COVID results from recent admission were negative. Of note patient did have recent contact with COVID positive individuals. 7/26/21: patient discharged

patient developed severe chest and epigastric pain that over the following two months began to involve her joints. She was evaluated in the ER multiple times and finally hospitalized and diagnosed with SIRS (systemic inflammatory response syndrome) and will undergo additional evaluation with rheumatology.

I had the shot and came home. That day I started to feel fatigue. The next 2 days after the shot. I was exhausted, queasy nauseated and the pain in my shoulder and in my armpit. this was two days later and felt under my armpit and looked in the mirror and cupped a large ball of swelling. after a few hours and days, it continued to get larger. I still was nauseated and couldn't sit. I took a photo of it. It took about three weeks to go down some. It is still not normal. I showed the photo to my doctor, and he was shocked. Now it puffy and I can cup it in my hand. I got the shot from the guy at pharmacy. My friend thinks I got in the bone. It still is not normal.

Dizziness; Weakness; Chills; Fatigue; Swelling of the neck; Vertigo; This is a spontaneous report from a contactable consumer, the patient. A 31-year-old male patient received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: EN0153) via an unspecified route of administration in the left arm on 07Apr2021 at 09:30 (at the age of 31-years-old) as a single dose for COVID-19 immunisation. Medical history was not reported. Concomitant medications included fish oil (OMEGA 3) and cyanocobalamin (B 12), both taken for unspecified indications from unknown dates. Prior to the vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had been tested for COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. On 07Apr2021, the patient experienced dizziness, weakness, chills, fatigue and swelling of the neck. On an unknown date in Apr2021, two weeks later, the patient experienced vertigo; fatigue and vertigo were present at the time of this report. The events resulted in doctor or other healthcare professional office/clinic visit. It was unknown if any therapeutic measures were taken as a result of the events. On 19Apr2021, the patient underwent COVID-19 test (nasal swab) and the result was negative. The clinical outcome of the events dizziness, weakness, chills and swelling of the neck were resolved with lasting effects on an unknown date in Apr2021, while fatigue and vertigo were not resolved at the time of this report. No follow-up attempts are needed. No further information is expected.

A 25-year-old male patient received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: EN0153) via an unspecified route of administration in the arm left on 06Apr2021 at 15:00 (at the age of 25-year-old) as a single dose for COVID-19 immunisation. The patient's medical history included COVID-19 from an unknown date to unknown date. Concomitant medications included restylane filler (MANUFACTURER UNKNOWN) started on 04Feb2021 for unknown indication. The patient had no known allergies. Prior to vaccination, the patient was diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. On 09Apr2021, the patient experienced dry hive like patches on arm which extended to back, chest, and torso. The event did not result in doctor or other healthcare professional office/clinic visit, and emergency room/department or urgent care. Therapeutic measures were not taken as a result of this event. The clinical outcome of the event dry hive like patches on back, chest, and torso was not recovered at the time of this report. No follow-up attempts are needed. No further information is expected.

A 32-year-old non-pregnant female patient received BNT162B2 (BNT162B2, Formulation: Solution for injection, Lot number: EN0153 and Expiration date: unknown), dose 1 via an unknown route of administration, in left arm on 20Apr2021 at 13:45 as single dose, dose 2 via an unknown route of administration, in left arm on 20Apr2021 (Batch/Lot Number: EN0175) as single dose for COVID-19 immunization (at the age of 31-years-old). The patient's medical history included hypothyroidism. Concomitant medication included levothyroxine sodium 88ug taken for an unspecified indication. The patient had no known allergies. The patient did not receive any other vaccines within 4 weeks prior to the COVID-19 vaccine. The patient was not diagnosed with COVID-19 prior to vaccination. On 23Apr2021 at 10:00, the patient experienced pain and swelling of supraclavicular lymph nodes. The patient was not tested for COVID-19 since the vaccination. No treatment was received for events. Outcome of events was not recovered.

Blood flowed into the throat before out the nose; This is a spontaneous report from a contactable consumer, the patient. A 23-year-old male patient received the second dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: EN0153) via an unspecified route of administration in the left arm on 13May2021 at 19:45 (at the age of 23-year-old) as a single dose for COVID-19 immunisation. The patient's medical history included sport induced asthma, (3/10) no inhaler prescribed and allergy to erythromycin since an unspecified date. The patient's concomitant medications were not reported. The patient previously received erythromycin (MANUFACTURER UNKNOWN) and experienced drug allergy on an unknown date. The patient also received previously the first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: EN0153) via an unspecified route of administration in the left arm on 22Apr2021 at 19:45 (at the age of 23-year-old) as a single dose for COVID-19 immunisation. Prior to the vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. On 14May2021 at 19:00, the patient experienced mild (4/10) (unit unspecified) nose bleed 5/14, within the bounds of being relatively normal and severe/worse (6/10) nose bleed 5/15, low viscosity/high flow rate. Blood flowed into the throat before out the nose. Both times nose bleed lasted under 2- 3min. The patient was advised via phone call from nurse and physician. Therapeutic measures were taken as a result of adverse event which included treatment with unspecified medications. The clinical outcome of the event mild (4/10) nose bleed 5/14, within the bounds of being relatively normal and severe/worse (6/10) nose bleed 5/15, low viscosity/high flow rate. Blood flowed into the throat before out the nose was unknown at the time of this report. No follow-up attempts are needed. No further information is expected.

Symptomtext

Profuse menstrual bleeding that has still not stopped as of this date. I have had an IUD since January 2012 and only have two or three episodes of light spotting each year, only last a day or two each. I have now been bleeding heavily since the night after I received my second dose and it?s not decreasing at all.
