A 58-year-old male patient received bnt162b2 (BNT162B2), administered in arm left, administration date 09May2021 10:15 (Lot number: EW0168) at the age of 58 years as dose 2, single for covid-19 immunisation. The patient had no relevant medical history. Known allergies: none. Concomitant medication(s) included: AMLODIPINE; CANDESARTAN. No other vaccine in four weeks. Vaccination history included: Bnt162b2 (Dose 1, Batch/Lot No: ER8731, Location of injection: Arm Left, Vaccine Administration Time: 10:00 AM), administration date: 18Apr2021, when the patient was 58 years old, for Covid-19 immunisation. The following information was reported: BELL'S PALSY (medically significant) with onset 07Dec2021, outcome "unknown", described as "Suffered facial neuritis or Bell's palsy". She told she doctors that she had just received the second dose of the covid vaccine. At the emergency room, they did a CT of her brain, a X-Ray of her chest, and blood work. They took her vital signs, and asked her a bunch of questions. Everything came back okay. She remembers getting the injection at 10:50, and then starting to have a reaction and looking at the clock and thinking, No follow-up attempts are possible. No further information is expected.

A 14-year-old male patient received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: EN0168) via an unspecified route of administration in the left arm on 15May2021 at 12:00 (at the age of 14-years-old) as a single dose for COVID-19 immunisation. Medical history included COVID-19. The patient had no known allergies. The patient did not receive other medications in two weeks of vaccination. Prior to the vaccination, the patient was diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. On 16May2021, the patient experienced fever, chills, tired, arm red and swollen around shot, swelling of armpit. The events did not result in doctor or other healthcare professional office/clinic visit, and emergency room/department or urgent care. The patient did not receive any treatment for the events. The clinical outcome of the events fever, chills, tired, arm red and swollen around shot, swelling of armpit recovered on an unknown date in May2021. No follow-up attempts are needed.
