The reporter is the patient. A 61 year-old female patient received bnt162b2 (BNT162B2), administered in arm left, administration date 09May2021 15:30 (Lot number: EN0179) at the age of 61 years as dose 2, single for covid-19 immunisation. Relevant medical history included: "Addison's disease" (ongoing), notes: she has had this for a while but was just diagnosed with it 2 years ago.; "Hashimoto's disease" (ongoing), notes: She says she has had this condition for about 6 years.; "nausea issues" (ongoing), notes: She says she has had this condition for at least 6 years.; "Metabolic syndrome" (ongoing), notes: blood sugar problem but it's not diabetes and body sometimes doesn't use insulin right for 6yrs now; "blood sugar problem" (ongoing), notes: blood sugar problem but it's not diabetes, and body sometimes doesn't use insulin right for 6yrs now; "hip arthritis" (ongoing), notes: she has had right hip pain for at least 2 years, clarifies it as arthritis in her hip.; "allergy to latex" (unspecified if ongoing). Additional information: No Prior Vaccinations within 4 weeks of Covid-19 Vaccine. No Additional Vaccines Administered on Same Date of the Pfizer Suspect. Patient (Pt) received her second Pfizer Covid 19 Vaccine on 09May2021. Reason for taking Pfizer COVID-19 vaccine reported as she wants to be able to go out and not worry about getting sick. Within 3-3.5 hours after the vaccine she experienced a dull headache, fatigue, diarrhea, soreness under her left armpit, nausea and decrease appetite. Pt was asking if she should get tested for Covid since the symptoms have been ongoing for 6 days now (at the time of report), reported that she might have to go to the clinic to get a quick test. No Emergency Room or Physician Office visit for events. Pt says she had her second dose on Sunday, clarified as Mother's day, and ever since that second dose she has had diarrhea bad, a dull headache, and she's been tired. She looked it up and she thought it would only last a day or two but this is going on 6 days tomorrow. The caller doesn't know if she should go have a test, she doesn't know what she should do. Pt says the diarrhea occurred on the same day as the shot when she got home. She says she was at her appointment at 3:30, went to her mothers and went home at 6:00 or 7:00, so it was probably 6:30 or 7:00pm that night that the diarrhea occurred. She says the headache was off and on; last night it was a little bit worse than it is today. She says along with the headache, she noticed under her left arm where she got the shot it felt like it was sore under and in her armpit. She says maybe some lymph nodes were swollen under her arm, she doesn't know, but that has gone away now. She says the sore under her left armpit occurred on the 10th or 11th, and went away last night some time. She clarifies this probably began 11May2021 because it was mainly sore where the shot went in and after that it hurt to put her arm down so she kept them up in the air. She says she seem to read online that the Pfizer, or maybe all the vaccines, have been liked with people having problems with their mammograms so maybe the problem is swollen lymph nodes. She says the outcome is unknown because it depends on the next mammogram. She says the tiredness occurred probably around 5:00pm in the evening on 10May2021. She says the nausea and low appetite started around 2:00pm on 10May2021 and she didn't feel like eating. She says coffee, which she usually has every day, really bothered her. She says the low appetite has improved slightly. Patient was taking Hydrocortisone for roughly 2 years, Armour Thyroid and Zofran for about 6 years. Pt says she has Addison's disease/ adrenal insufficiency; her adrenal glands don't work. With the hydrocortisone, she confirms she takes 10mg currently but she started on 20mg but sometimes she has to up-dose to 25 to 30mg or more depending on what stressors she has in her life. For example, if she has surgery, she says she might up-dose to 100mg. She explains she has to divide the doses up like 10mg in the morning, 5mg in the afternoon, 5mg at bed, so she is keeping her adrenal glands working like they are supposed to. Depending on the dose she has to take that day, the total dose of 10mg or 20mg or whatever she is taking, is divided up throughout the day. he says she is on many medications because of her health. She says that's another reason to be vaccinated because the say it is important if you have a weak immune system to get it. She says she was afraid to get it because she was afraid of how her immune system was going to act. She says she had a hip injection between shot number 1 and shot number 2. She says they went in and injected her hip joint with a steroid, some dye, and some lidocaine. She says she had a really bad reaction to that and she had to go to the hospital in an ambulance because she was just in pain. The caller clarifies the steroid medication was cortisone. She says she does not know the dose, NDC number, lot number, or expiration date of the steroid cortisone because they put her under for the procedure. She does not know the dose, NDC number, LOT number, or expiration date of the lidocaine. She clarifies there is something going on in her hip; she has arthritis in her hip and she was trying to get pain relief without doing hip surgery. She has been experiencing right hip pain for at least 2 years. She confirms that the onset of the right hip pain was well before either vaccination dose. She clarifies she went to the hospital for a scheduled surgical procedure by an orthopedic surgeon and she was not admitted to the hospital after receiving the injection. Pt had a reaction to one of the flu vaccines many years ago and since then she has never gotten another one. She says that was probably about 10 years ago. She does not know the NDC number, LOT number, or expiration date of that flu shot. She clarifies her reaction was she got sick with flu-like symptoms and was down for a few days. She says she wasn't prepared for that. Pt denies being sick at the time of either vaccination. Pt says the levoxthyrozine reaction was probably about 4 years ago. She does not have an NDC number, LOT number, or expiration date for the levoxthyrozine. She says she might have the bottle but she'd rather not go get it at this time. She clarifies her reaction to the levoxthyrozine was that she was extremely tired, she slept all the time, and her blood pressure was dropping, her body temperature was dropping. She says her primary care physician said to get off it because the endocrinologist didn't think it was fine but her primary care physician said something is wrong and to get off of it. She says as soon as she got off of the levoxthyrozine, she felt better. Pt was a caregiver for people in homes and in retirement communities. She clarifies she was not a registered nurse, just a caregiver. She declines to give her email address. She says she never goes on her email. She does not give an email address for her primary care provider. No follow-up attempts are possible. No further information is expected.
