11/22/2021: Patient tested positive for COVID in outpatient/community setting 11/23/2021: given Casirivimab-Imdevimab dose in outpatient/community setting 11/24/21 @0100: Presented to ER post fall with head laceration. Noted to have SBP 80's, MAP <50, RR 30-40, HR 130-140, sat 73% on RA with respiratory distress on movement and accessory muscle use at rest. Rapidly declining status required intubation in the ER. BUN Crt 29/1.4, EGFR 51, WBC 15.7, CRP 8.3, troponin 0.210, glucose 304, ABG 7.331/35/143/18.5/BE -6.7 sat 100% on Bilevel vent settings 30/10 PS 20 FiO2 100%. EKG shows new depression in inferiolateral leads. CXR diffuse interstitial opacities consistent with COVID 19 PNA. Admitted with acute hypox hypoxemic respiratory failure in the setting of bilateral COVID 19 PNA, septic shock, AKI, and NSTEMI with troponin peak of 0.483. Started on IV Rocephin, IV doxycycline, methylprednisolone, baricitinip, levophed, IVF, ASA, and heparin gtt. 11/26/21: Heparin gtt changed to Lovenox. Bilevel 14 20/8 Fio2 80% WBC 24.2. FMS placed for diarrhea c.diff negative. Levophed weaned down to maintain MAP >65 SBP >100. 11/30/21: Started PSV weaning trials in am. Extubated in PM to 4LNC. After a few hours post extubation required 8L via NC to maintain sat >90%. RR 20's 12/1/21: Increase to 10L via HFNC RR 30's HR 120's. Patient wished to have code status changed from full code to DNR/DNI with trial BiPaP. 12/2/21: 15L via HFNC with intermittent BiPap 14/8 FiO2 70% to rest. RR 30-40 HR 110-120 anxious and dyspnic with any activity or movement 12/3/21: Requiring BiPap at all times 100% FiO2. Increased respiratory distress and anxiety. Family meeting with patient held. Decided on comfort care measures only. Placed on 4L NC, morphine and ativan gtt for comfort. 12/6/21 @2330 patient expired on comfort measures.

This is a spontaneous report from a contactable consumer (patient himself). A male patient of an unspecified age received second dose of BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Formulation: Solution for injection, Lot number: EN1699, Expiration date: 15Mar2021), via an unspecified route of administration in left arm on an unspecified date as a single dose for COVID-19 immunization. The patient's medical history included high blood pressure, football injury to his knee. Concomitant medication included unspecified medications. Patient stated, "yeah, he take the lowest dose for something for high blood pressure". The patient previously received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Formulation: Solution for injection, Batch/lot number: EN6198), via an unspecified route of administration on an unspecified date as a single dose for COVID-19 immunization. No other vaccine (4 weeks prior). It was reported that, it's been couple of months since he had his second injection. Patient have his card with the numbers but 2 weeks after the second injection, he had like muscle cramps in his entire leg for like two weeks, it was like an internal withheld has waken up and it would not go away. Patient could find overly until he actually discovered a position that would satisfy whatever that strange thing it was, it felt like his leg was swollen but there was no visible swelling. The tension on his skin was numb like he have been kicked in the shins, nothing was swollen but it felt numb. Patient bent over and touched his toes and the flats of his palms of his hand on the floor. Patient got into resistance and that pain went away but for 2 weeks patient consumed about two thirds of a bottle of the Tylenol (treatment) and patient talk to a lot of customers who, he have been back to work now from a month, everything was opening up and they have complaint the worst or similar side effects (further information unavailable) and that's what make him say that may be it was worth giving you guys a call just to let you know to know he thought, he should not go to the hospital for this. That was the only alternative to give call on withheld but still he said Jesus, feels like he ripped his aorta which was bad (further not clarified. Hence, not captured in tab) and he says withheld, and it didn't go away. It wasn't just isolated to his knee which he had a football injury when he was in high school to his knee and when the weather was bad his knee hurts and he figured that's what it was, it started out like that and it progressed to something and went, and it was lot more difficult to manage. Patient just want to pass it along for what's it worth, so you guys have the information. Still experiencing: Patient stated, "no, it's gone, it's gone since the first week of May2021, but patient would like to tell that he was going to tell you about he had no strength in legs. Patient was telling you about, he had no strength in it. Patient almost walked down the stairs he was like Holy Jesus, it was better but it's still not where it used to be in terms of the strength in the right leg. Patient still act like he was mad lymph, but he could feel like little relaxation and it's getting better, but he don't think it's permanent thing. Patient dread the idea of the booster." Patient stated, "Tylenol 500 mg, no more than 6 a day but they helped. Patient would say that they helped but not enough." No investigation assessment. The patient was treated with Tylenol 500 mg for pain, when the weather was bad his knee hurts, it progressed to something. The outcome of the event "had no strength in legs/ it was better but it's still not where it used to be in terms of the strength in the right leg" was recovering and unknown for the other events. Follow-up are needed. Information on the lot/batch number has been requested.

I woke up on March 5, 2021 before daylight back of my throat sore, chest congestion, felt like a minor case of COVID. The symptoms was very similar to COVID expect I had a bad headache. I went to my doctor seen another doctor in the practice. She diagnosed me with Bronchitis prescribed me a antibiotic. When I seen my regular doctor informed him of what happened.

woken up with swollen his left side of his face and his cheek near his jaw whole side was significantly swollen; Fever; Diagnosed him with salivary gland infection; This is a spontaneous report from a contactable consumer. This consumer (father) reported that a 14-year-old male patient received bnt162b2 (BNT162B2, Formulation: Solution for injection), via an unspecified route of administration, administered in Arm Right on 23Jun2021 16:00 (Batch/Lot Number: EN1699) as dose number unknown, single (at the age of 14-year-old) for covid-19 immunisation. Medical history included autism spectrum disorder from an unknown date and unknown if ongoing Verbatim: Autism spectrum. Concomitant medications included risperidone, guanfacine. Reporter was calling on behalf of son he was 14-years-old and so he was autism spectrum was non verbal. The patient received the vaccine at 4pm on 23Jun, the next day which in 24th in afternoon 3 pm he developed a fever which was around closed to 100 degrees Fahrenheit and then we gave him Tylenol (Further clarification was unknown) reporter thought because of the vaccine shot. On next day morning he had woken up with swollen his left side of his face and his cheek near his jaw whole side was significantly swollen. So, we immediately took him to the urgent care. Reporter stated, no laboratory test done, doctor basically put him on antibiotic Amoxicillin (Treatment) for the salivary gland infection diagnosed him with salivary gland infection they put him on Amoxicillin and swelling subsided in 3 days and fever went in 2 days and there was no further analysis needed. Therapeutic measures were taken as a result of diagnosed him with salivary gland infection, fever, woken up with swollen his left side of his face and his cheek near his jaw whole side was significantly swollen. The outcome of the event woken up with swollen his left side of his face and his cheek near his jaw whole side was significantly swollen was recovered on 28Jun2021, event fever was recovered on 26Jun2021 and diagnosed him with salivary gland infection recovered in Jun2021. No follow-up attempts are possible. No further information is expected.

Pt's daughter called around 4:30pm on 3/11/21 (day 2 post vaccination on 3/9/21) reporting that pt woke up feeling nauseous all day & vomited profusely at 4:00pm. Advised to stay hydrated and take nausea medication patient had at home & to call back with any changes. If not improved in 6-12 hours, advised to call physician for evaluation.
