COVID-19 Hypoxia Intensive care SARS-CoV-2 test positive

Symptomtext

Patient received one-dose of Pfizer COVID-19 vaccination on 3/10/21, subsequently tested positive for COVID-19 on 3/25/2021. Due to increased oxygen requirements, had to be admitted to the Health Care System on 3/26/2021. Patient was started on dexamethasone and remdesivir inpatient, but still developed worsening hypoxia requiring ICU admission to maintain adequate oxygenation. Highest oxygen requirement was 15 L/min. At the filing of this report, patient remains in the ICU on 6 L/min of oxygen via nasal cannula.

Symptomtext

Patient immediately began felling unwell after vaccination. Patient felt "generally unwell" and in a daze. Never alleviated, but second dose received. Patient then developed hypertension, tachycardia and was diagnosed with Afib within 2 weeks of vaccination. Patient was hospitalized and a pacemaker was placed.

~8 minutes after receiving her vaccine, pt approached pharmacy complaining of racing heart / sweating / mild trouble breathing / feeling faint, after assessing pt 1 dose of 0.3 mg epinephrine was administered. paramedics were called to complete a full work up of pt. After checking her vitals and monitoring heart rate, pt opted not to proceed to ER and go home instead.

Symptomtext

Slight sharp burning pain "behind" left eye begin a few hours after I had shot. That pain was intermittent for a few hours and dissipated by early evening of the day of the shot. My left lip and left side of throat had minor feeling of tingly burn when I was driving home 30 minutes after I had the shot. Stiff muscles on left side of neck. Intermittent mild muscle pinching along left side of body but went away by Friday night.

2 Events: Evening following 2nd injection, I went from having no dental issues to having severe tooth pain on lower left jaw, roughly 4-5 teeth in from back molar. Swelling began within hours accompanied by intense pain and sensitivity. By that Sunday evening, swelling had extended up to left eye, over to bottom of ear, and down neck to baseline and half way to center jaw line. Bruising present. Throbbing was constant. Made emergency dental appointment for Monday, but by that Sunday night, I could not stand it any longer and was afraid the swelling would extend to my throat. Went to ER, who diagnosed an abcess under the tooth root. Perscribed pain pills and antibiotics. Saw dentist Monday, who scheduled me for an emergency root canal on Tuesday. Got root canal on a tooth I never had problems with before, although there certainly could have been an issue brewing but not yet pronounced. Anyway, he also drained what he called an unbelievable amount of infection fluid (pus) after making an incision in my gum line and basically squeezing my face to get as much out as he could. Said he never saw anything like it. And for me to go from 0 to that in a matter of days was incredible. I thought perhaps it was an immune response? At the same time that I was experiencing the dental issue, I started to develop a strong dry cough. I have a light former smokers cough, and have had pneumonia many times, but this cough was different. Every day it has gotten strongly, but is very much intermittent. I now go into fits of coughing that last one to two minutes and so strong and to the point where I have either fractured my rib directly beneath my right breast, or something else is going on. These fits can last minutes, but never result in expulsion of phlegm. The coughing fits also occur if I take a deeper breath, walk too far, laugh, or talk too long. Very strange. I am very fatigued and have been since the onset of the dental issue reported above. While the dental issue is gone since the absess was drained, the cough is still present, and wakes me up every single night too with coughing fits. The cough has loosened up a bit. I would go to the doctor, but I am in the middle of a move from MO to AZ, and just arrived in AZ after a 3 day drive. So, first off, I am reporting this issue. And next, I will seek a new doctor in Arizona if this cough continues.

Symptomtext

Patient expressed light-headedness. Vitals taken and patient observed. Patient had not taken blood pressure medication in the morning, and was on way home to do so. BP was coming back down by 9 am. Patient continued to feel better and at 9:15 went home to take medication.

Dizziness Feeling hot

Symptomtext

Patient reported felling hot and dizzy. Pt was alert and oriented. Patient was relocated to clinic room bed and placed in supine position.. Extremeties were elevated. Pt began to report feeling better almost immediately. Blood pressure was obtained at 1:03pm. BP was 146/96, HR 55. Pt was instructed to remain in reclined position. She was alert and conversational throughout. At 1:10p, another BP was obtained of 138/68. HR 58bpm. Pt continued to report feeling better. She was able to sit up and remained seated for several minutes to ensure no orthostatic hypotension. She was able to stand and left clinic at 1:16pm. Impression of vasovagal reaction post injection.
