Pfizer COVID Vaccine EUA: COVID-19 case resulting in Hospitalization / Death. Patient received Pfizer Vaccines on 01/23/21 and 02/13/2021. COVID + on 1/10/22. Presented to ED on 1/17/2022 with acute hypoxic respiratory failure 2/2 COVID-19 and needed 2L on admit. Presented with malaise and SOB x 1 week. Previously admitted on 1/10/22 and treated with Decadron. CXR showed worsening bilateral pulmonary opacities given Lasix and also received supplemental O2 via HFNC +NC. Code status changed to DNAR. Patient expired on 1/25/22

Had a stroke a week after the first vaccine; His left side is nonfunctional/ his left side never came back; He almost died; Muscle spasm; He is not able to walk; This is a spontaneous report from a contactable consumer (patient's wife). A 67-years-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, formulation: Solution for injection, Batch/Lot Number: EN5813) via an unspecified route of administration on 30Jan2021 (at the age of 67-years-old) as dose 1, single for COVID-19 immunization. The patient's medical history included rheumatoid arthritis, diabetes both from an unknown date and unknown if ongoing, he was diabetic, he was borderline diabetic so he was controlling it from diet. Concomitant medications included methotrexate taken for rheumatoid arthritis, start and stop date were not reported. On 10Feb2021, the patient experienced had a stroke a week after the first vaccine, in 2021 patient experienced he almost died, muscle spasm, he was not able to walk, his left side was non-functional/his left side never came back. Reporter was taking care of her husband 24/7 at home, his left side was non-functional. Reporter stated, his blood pressure pretty normal, of course after he got the, after he had the stroke a lot of stuff went. He was in the hospital for 30 days, he almost died. It had not did any improvement on the left side. Seriousness criteria reported as caused/prolonged hospitalization. The patient was hospitalized for had a stroke a week after the first vaccine (cerebrovascular accident) from 10Feb2021 to Mar2021. Reporter stated, 10Feb2021 was when it happened, he went right in and he stayed there 30 days and then he went to rehab for another 30 days so, 10Feb2021 she would say, Mar2021 he came out of the hospital around in middle of the Mar2021. He was not able to walk, his left side never came back. Reporter seriousness for other events was unspecified. Reporter stated, He was on a blood thinner Eliquis, which was very expensive but other one for that so we have to keep him on that, he was on, they put him on the cholesterol medication to keep his arteries clear and they did a procedure with an open duct the blockage and they took care of that, but by the time they did that the damage had been done so and he was also on Baclofen for his muscle spasm (further clarification unknown). Outcome of the events he was not able to walk, his left side is non-functional/ his left side never came back was not recovered and for all other events was unknown.

Breakthrough COVID first vaccine 1/6/21. Sent due to positive COVID and shortness of breath/dyspnea on exertion. Patient has had progressive shortness of breath for approximately 3-4 weeks. It appears that her shortness of breath per minute her from going to the dining hall at night. According to the daughter, patient's shortness of breath has gotten progressively fever worse over the past 2 days. Patient was checked for COVID today and she reportedly was positive. Due to her positivity and shortness of breath she was sent to the ED for further evaluation. Once in the ED, patient was found to have a saturation of 90-92% at rest. Patient denied fever, chills, cough.

Under the site where I was given a shot above that I have a rash; Flu like symptoms; Fever; This is a spontaneous report from a contactable consumer (patient). A 67-year-old female patient received second dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE; Solution for injection; Lot number: EN5813) via an unspecified route of administration on 10Feb2021 Wednesday (at the age of 67-year-old) as single dose for COVID-19 immunization. Medical history and concomitant medications were not reported. Patient previously received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE; Solution for injection; Lot number: unknown) via an unspecified route of administration on an unspecified date as single dose for COVID-19 immunization and had no reaction. On 11Feb2021, Thursday, patient experienced flu like symptoms, and had little bit of fever, that day and went away yesterday on 12Feb2021. At night on 12Feb2021, patient noticed that not directly, under the site where she was given a shot above that she had a rash and that was her concern, but it was not all over her body just under my arms not directly below the site but below it. Patient was taking Benadryl in response to the event rash. Outcome of the event flu and fever was resolved on 12Feb2021 and for rash was unknown. Follow-up attempts completed. No further information expected.

nausea; vomiting; dizziness; rapid heart rate (139 beats/min)/heart rate was elevated; This is a spontaneous report from a contactable pharmacist. A 46-years-old female patient received the first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Formulation: Solution for injection, Lot number: EN5813), intramuscularly in the left arm on 02Feb2021 at 11:00 as a single dose for COVID-19 immunization. The patient's medical history included history of reacting to flu vaccines and flu-like symptoms. The patient's concomitant medications were not reported. It was unknown whether the patient was pregnant at the time of the vaccination. It was unknown whether the patient received any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, the patient diagnosed with COVID-19. It was unknown if the patient tested positive for COVID-19. It was reported that, on 02Feb2021 at 11:00 PM, approximately 12 hrs after inoculation, the patient experienced nausea, vomiting, dizziness and rapid heart rate (139 beats/min). The patients heart rate was elevated for 2 hrs. The treatment received for the events was unknown. On 02Feb2021, the patient underwent lab tests and procedures which included heart rate and resulted as 139 beats/min, rapid. The outcome of the events was recovered. The reporter classified the events as non-serious and seriousness criteria that resulted in death, life threatening, caused/prolonged hospitalization, disabling/incapacitating and congenital anomaly/birth defect was reported as no. Follow-up attempts are completed. No further information is expected.
