Type 2 diabetes, requiring insulin, diagnosed in 2000, was followed by Dr and was on an insulin pump on until mid 2016 when his pump broke. Hyperlipidemia. HTN. CAD requiring coronary artery stenting 2 in 2015/2016. Obesity. Chronic arthralgias/myalgias with thoughts of potential fibromyalgia. C.difficile. Mild congnitive dysfunction after MVA . Motor vehical accident in 1989. BPH. Hypogonadism. Hearing loss bilaterally but no testing so far . Tinnitus since 2000. Testosterone deficiency.
