On Tuesday, July 13, 2021, I went to bed after feeling tired and woke up a 3 am with a terrible headache, dizziness and when I stood up, I had a total loss of bodily awareness, unable to control my motor functions or orient myself to my surroundings. I managed to crawl back to bed and slept. I awakened at 6 am with a slight headache, low energy level, and nausea. I called and saw my PCP on 7/16/2021 to discuss what happened. My PCP ordered a CT scan. The diagnostic IMPRESSION: Acute-subacute right cerebellar infarcts which have developed since 6/25/2021. Cerebral and cerebellar parenchyma: There is new hypodensity seen within the right cerebellum compared with the prior CT examination on 6/25/2021. Cerebral infarction due to embolism of the right cerebellar artery. I was then transported to Hospital ER for further evaluation and monitoring. Hospitalized 7/16/2021-7/18/2021 and underwent more neurological testing.

I requested a referral to cardiology from my PCP for ongoing heart palpitations, dizziness, and shortness of breath. I was seen on 3/18/21 by, MD in Cardiology. I explained I had an Echo done in 2015 that showed moderate left atrial enlargement, and a more recent EKG on 11/30/2020 prior to scheduled surgery that was suggestive of left atrial enlargement. Concern was over if I was at risk of atrial fibrillation. Testing was started. Follow-up after testing with PA in Cardiology on 5/17/2021. The previous work-up for palpitations included a Holter monitor (as opposed to a Zio patch because I am allergic to adhesives) which revealed symptoms of chest pain correlating to normal sinus rhythm. Holter and echo unrevealing, negative treadmill stress test in April. No significant wall motion abnormalities or valvular disease. The echocardiogram revealed normal LV function with basal septal hypertrophy without evidence of LVOT obstruction. No significant wall motion abnormalities or valvular disease. Started a new diuretic medication chlorthalidone and increased Lisinopril to 40 mg from 20 mg. I was unable to tolerate the chlorthalidone due to a rash and lowered the Lisinopril to 20 mg. The next appointment with cardiology is scheduled for November 17, 2021. This was a major event that should have killed or disabled me. Instead, no one can explain how I am here. I was also seen and evaluated by, MD on 7/22/2021 NEUROLOGY CONSULTATION. Her assessment of my history of bilateral cerebellar strokes is, fortunately, resulting in no increased pressure in the posterior fossa, despite a large area of infarctions from the films that were reviewed. I have minimal deficits if any, mainly relating to tracking and a mild lingering headache that responds well to turmeric. MRA of the head and neck were done and a referral to PT and OT. I was to discontinue Plavix after 3 weeks and continue with a daily low aspirin regimen. I am also now on 80 mg or atorvastatin for life.
