06/15/22 Had a temp of a 100.xx that evening. On 06/16/2022 left deltoid arm area was swollen and hot to touch. The area affected was approximately 4 inches in an oval shape. There were pin point blisters. The child c/o it itching a bit. 06/17/22 the swelling continued to reduce. 06/20/2022 went to see doctor for poison ivy on face and received a steroid. Mom showed the doctor pt.'s arm and the doctor was not concerned about the vaccination or arm area at that time. 06/21/22 mom said pt. is doing pretty good, no swelling at this time, still a little hard but no redness.

She began with a rash with red whelps, with eyes red and puffy, starting with one and then the other. Her hands became erythematous and puffy. She did not have cough wheezing, hoarseness, or difficulty breathing. She was advised to give a 5 ml dose of benadryl every 4-6 hours and to go to the ER if her symptoms worsened. Called back the next day to follow up. Mom had given benadryl schedule for 24 hours, and she had improved. The first 24 hours, the rash was constant and she had to keep her on hte benadryl. On a visit with her on 3/14 (2 days later), mom reported she was continuing to develop whelps, white and red raised areas that started at her feet , moved up and then went down, multiple cycles. On 3/14 this happened 4 times. She had stopped the benadryl after the first day. Mom noted the shot sites were still swollen as well. The rash had also flared after eating some ice cream that she had never reacted to before. Mom had had to use albuterol just that day for some wheezing. She was started back on benadryl and famotidine was added as well as a prednisolone taper.

1 week later presented with petechia to the left lower thigh and upper leg; pinpoint non-blanching lesions purple in color. No other sites of petechia or clinical bleeding. CBC was done and normal including platelet count of 489mm3. He returned to clinic 1 week later with nearly resolved rash.

Infant patient received an IM dose of Prevnar 13 and Pentacel on 02/09/2022. Several hours later he developed a fever of 103.5. A fever of >102 persisted for 4 days until 02/13/2022. Notably, he had no viral symptoms of any kind (ie. no cough, runny nose, or congestion) during the entire period of the fever. He fed well and had ample urinary output. On the day that the fever resolved he developed a blanching erythematous rash on his torso, limbs and face. His mother states that his skin is extremely sensitive to foods and she had given him Motrin suspension containing orange dye prior to the onset of the rash.

A FEW SECONDS AFTER BEING GIVEN THE PREVNAR VACCINE, THE INFANT HAD A PAUSE IN HER BREATHING FOR 5-10 SECS. IT APPEARED AS THOUGH HER "EYES ROLLED BACK TOO". MOTHER THOUGHT SHE FELT LIMP DURING THE EPISODE. MA DESCRIBED HER AS BEING MORE STIFF. I WAS CALLED TO THE ROOM IMMEDIATELY AND BY THE TIME I ARRIVED, THE INFANT WAS ALERT, LOOKING AROUND AND BREATHING WITH NORMAL EFFORT AND RATE. LUNGS CONTINUED TO SOUND CLEAR. I suspect the episode was related to the pain/discomfort of the Prevnar injection.

Infant developed hives ~ 3 hours after vaccination. Noted only on lower extremities on the day of vaccination 2/16/22. No hives on body or face, no lip or eye swelling, no coughing, runny nose, or congestion. She started vomiting when she was given solids at dinnertime on day of vaccination. Mom notes coughing preceded vomiting. Infant able to keep her formula and liquids down without vomiting. Yesterday, noted to be "weaker", not wanting to walk as much, hives worsened on the legs. No hives on the body noted. Still without fever. Infant seen in the office today (2/18/22) @ 12pm with 1 small area of urticaria on left lateral thigh, few flat patches on lower leg bilaterally. No wheezing noted, no lip/eye swelling. Infant was fussy, but consolable. No altered mental status appreciated on exam.
