10/4/2022-Presents to ED via EMS, with reports of AMS, only responsive to painful stimuli. Ordered ceftriaxone, UTI. Trop peaked at 0.144 consistent with a Type 2 NSTEMI, recieved asa and on Heparin drip. Covid - test. Recent admission 9/11-9/23/2022 for acute hypoxic resp failure PNA. VSS 02 94% on RA. CXR-bibasilar airspace disease. Admit metabolic encephalopathy in setting of UTI. 10/7/2022-BP elevated 181/66 Hydralazine added. Desat 85% 4L via NC placed. Ceftriaxone changed to Unasyn, to cover for aspiration. 10/10/2022-UC yeast and VRE. restart oxybutin and start linezolid and fluconazole. Now covid +. 10/12/2022- Continue on bumex, isodil and Procardia. On RA VSS. CXR-b/l pulm infiltrates. Reanla dysfunction 10/12/2022- does not permit to use remdesivir or baricitinib. 10/13/2022- O2 increased to 6L nc after desat of 87%. Unasyn changed to Zosyn. Dexamethasone started. 10/14/2022-O2 increased to 8L NC. 10/15/2022- o2 increased to 15 L NRB, made palliative. Time of death 2056.

Headache Neck pain

Symptomtext

Constant headache; Severe neck pain; This is a spontaneous report from a contactable healthcare professional, the patient. A 43-year-old non-pregnant female patient received her second dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot number: EN9269), via intramuscular route in the left arm on 09Mar2021 at 12:30 (at the age of 43-years-old), as a single dose for COVID-19 immunisation. Medical history included anxiety and depression. The patient had no allergies to medications, food, or other products. Prior to vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Concomitant medication included fluoxetine (MANAFACTURER UNKNOWN) 40 mg for an unknown indication. The patient previously received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Lot number: EL9269), via intramuscular route in the left arm on 15Feb2021 at 12:00 (at the age of 43-years-old), as a single dose for COVID-19 immunisation. Since the vaccination, the patient had not been tested for COVID-19. On 10Mar2021, the patient experienced constant headache and severe neck pain that moves for over a week. The patient did not receive any treatment for the reported events. The clinical outcome of the events head ache and neck pain was not recovered. No follow-up attempts are needed. No further information is expected.

Malaise Pyrexia Tinnitus

Symptomtext

Pfizer vaccine, shots 1 and 2, one month apart. 2-24-21, and 324-21. I began getting tinnitus in both ears within a week or so after shot one. It really got annoyingly noticeable in both ears after shot two. I would consider the after effects of each dose to be normal...dose 1 felt a little lousy the next day with headache and fatigue but ok after that, dose 2 felt lousy the next day with low grade fever about 100.5. Felt fine the next day. Except for the tinnitus.

First shot - slightly tight feeling in my throat about an hour after shot, low grade fever second day Second shot - tight feeling in throat about an hour after shot for about 4 hours, and light headedness; fever later in day, fell asleep early slept 10 hours; second day ok in morning then fever getting higher all day but now better (6:30 pm so 33 hours after shot), extreme itchiness today day 2, like fleas - took Benadryl at noon and need more now; stomach ache and diarrhea.

According to onsite fire company client symptoms, 8:38 am-alert to name but couldn't recall year and date, confused, slow to react, slow speech, dilated pupils. 8:40 am-BP 145/78, pulse 78, O2 Sat 97%,. 8:42 am-Client verbalized feeling nauseous and dizzy but she was okay, pupils semi dilated, able to follow command. 8:43 am-Per paramedic client needs further evaluation. 8:45 am-BP 142/80, pulse 79, O2 sat 100%, Blood glucose 79. 9:01 am-rescue crew arrived on scene. 9:08 am-Client transfer to hospital for further evaluation due to in and out periods of confusion.
