66-year-old male admitted to Hospital for generalized weakness via EMS. Patient is COVID-19 positive with family exposure and has a past medical history significant for stroke, hypertension, residual right-sided weakness, hyperlipidemia, hypothyroidism, CHF, renal failure on peritoneal dialysis, anemia, and vascular dementia. At this time, patient is unable to provide any meaningful information in the way of HPI and phone number on file for family contact is out of service. On discussion with ED team, patient brought via EMS d/t generalized weakness, fatigue prior to arrival and medication noncompliance. He has been admitted for further evaluation and medical management.
