within three hours of injection shoulder pain, fever spiking as high as 102 degrees, fever with spikes of up to 102 lasting for a period of about 8 days. Nausea and vomiting on day 4 after vaccine injection, increasingly intense shoulder pain starting about 1 week after injection. Prescription of Acetominophen and Ibuprofen eventually resolved fever, but not shoulder pain. Sudden facial drooping - visit to emergency room and diagnosis of Bell's palsy- three weeks after injection, resolved after 12 day course of Prednisone. Currently ongoing left shoulder weakness, limited, reduced range of movement, loss of coordination and pain both when using and when resting left arm ( the injection site arm).

Beginning overnight after being vaccinated patient reports a bothersome redness and swelling at the site of vaccination that made moving her arm to certain positions uncomfortable and difficult. Patient took acetaminophen for the pain with marginal benefit. Patent reported to the pharmacy the night of 4/18/23 to report the reaction and says that today the swelling and soreness appears to be improving.

Patient reports that her right arm was sore and stiff the 1-2 days following the shot. More that when she gets flu shots, but not as bad as Shingrix shot, in her words. Then she began to notice more swelling, rash, and itching from shoulder blade, shoulder, arm down to right about elbow. She has not treated it, but at the time of reporting she said it was going down/getting better. Date she reported it was 8/24/23- 1 week after the vaccination. Pharmacist recommended contacting the Dr if the rash did not continue to get better or got worse again.

EXTREMITIES: R arm with 23 x 14.5cm area of erythema and increased warmth with 2-3 lightly excoriated areas from scratching, no evidence of infection or drainage. A patient (no qualifiers provided) received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), as dose number unknown, single (Lot number: GN1897, Expiration Date: 31Aug2024) for immunisation. The patient's relevant medical history and concomitant medications were not reported. The following information was reported: ERYTHEMA (non-serious), outcome "unknown"; SWELLING (non-serious), outcome "unknown", described as "swelling about one week after injection."; FEELING HOT (non-serious), outcome "unknown", described as "Felt hot"; FATIGUE (non-serious), outcome "unknown", described as "some fatigue". Therapeutic measures were taken as a result of erythema, swelling, feeling hot, fatigue. Additional information: One description from the doctor (for one of the patients) Erythema, swelling about one week after injection. No fevers or chills, no drainage, no sore. Felt hot, generalized swelling, well demarcated irregular area, no respiratory symptoms. some fatigue. The caller stated but she took Benadryl so she is guessing that is the fatigue thing.

Patient states that he had pain in the first days after vaccination, ranging up to an 8 out of 10. He is still having pain now, over a month later, but it is less severe. If his arm is at rest, he has no pain up to a 1 out of 10. When using his arm, especially lifting it up, he has pain described as a 5 out of 10. He has taken ibuprofen and used ice, to some result. He was seen by his doctor, who noted that there is no swelling, redness, infection, or anything else notable.

On 5/25/23 pt. seen in reported local reaction to vaccine. Pt seen in office on 5/25 by MD. MD reported at site of vaccine-- swelling, hard, red, tight, perhaps some SOB, no wheezing, no cough, no fevers, felt hives internally. MD recommended OTC antihistamine with icing for swelling, referred to allergy for evaluation. Allergy PA noted that local reaction was most likely due to the Prevnar 20 vaccine and that pt. can continue icing.

Patient reported feeling tired and nauseous as well as experiencing vomiting and chills the day of and day after vaccine. One day later the patient developed significant itchy rash and some swelling down the arm of the injection site. Rash localized to left arm. Patient has had similar reactions to other vaccines this way. Patient was instructed to report to the pharmacy and to her primary doctor any worsening of symptoms or if symptoms do not subside within a couple days. Patient was instructed to use ibuprofen to help with swelling and steroid cream if rash continues.

A 69-year-old female patient (not pregnant) received pneumococcal 20-valent conjugate vaccine (diphtheria CRM197 protein) (PREVNAR 20), on 20Apr2023 at 11:30 as dose number unknown, single (Lot number: GN1897, Expiration Date: Aug2024) at the age of 69 years intramuscular, in right deltoid for immunisation, pneumococcal immunisation. The patient had no relevant medical history. There were no concomitant medications. The following information was reported: DEVICE MALFUNCTION (non-serious), ACCIDENTAL EXPOSURE TO PRODUCT (non-serious), EXPOSURE VIA SKIN CONTACT (non-serious) all with onset 20Apr2023, outcome "unknown" and all described as "Prefilled syringe for PCV20 malfunctioned, causing all most of all of the medication to leak on the patient's arm"; DEVICE LEAKAGE (non-serious), POOR QUALITY DEVICE USED (non-serious) all with onset 20Apr2023, outcome "unknown" and all described as "as soon as we tried to press the medication out, all of the liquid leaked from the Luer lock. It seems the luer lock (slip tip) is defective in this prefilled syringe"; PAIN IN EXTREMITY (non-serious) with onset 2023, outcome "recovered", described as "sore arm". Additional information: It was reported that the reporter had a malfunctioning pneumococcal 20-valent conjugate vaccine prefilled syringe. It was stated the healthcare professional tried to vaccinate a patient today (20Apr2023), the needle was placed correctly on the patient's arm but as soon as they tried to press the medication out, all of the liquid leaked from the Luer lock. It seems that the luer lock (slip tip) was defective in this prefilled syringe. They have never had a prefilled syringe malfunction on us before and would like to get this one replaced. Upon follow up on 12May2023, the reporter described the medication error as "Luer lock malfunctioned, the vaccine leaked from the side as it was being administered to the patient." The medication error occurred when the product was administered to / taken by the patient. The factor considered to be responsible for the medication error was the Luer Lock malfunction. The product was prescribed by a healthcare professional and was taken as prescribed. The patient was given a PVZCO after the incident. The patient did not experience any adverse event(s) as a consequence of the error. The reporter additionally commented that the prefilled syringe for PCV20 malfunctioned on 20Apr2023, causing all most of all of the medication to leak on the patient's arm. The patient was given a second PCV20 shot, per providers recommendation and the patient agreed. They spoke to the patient on 03May2023 and she stated that she only had a sore arm, but was completely fine; no further side effects. The vaccination facility was the physician office; the vaccine was not administered at the facility. There were no other vaccines administered on the same date with the Pfizer vaccine considered as suspect. Follow-up (20Apr2023): This is a spontaneous follow-up report received in response to the mail trail sent regarding the confirmation of below mentioned query. Follow-up (12May2023): This is a spontaneous follow-up report received from the same contactable reporter (Consumer or other non HCP) in response to non-HCP letter sent. Updated information includes: patient details (added height and weight); medical history and concomitant therapy (none); suspect drug (added indication and vaccine facility information; updated vaccination time and anatomical location); reaction data (new events: "sore arm" and "Prefilled syringe for PCV20 malfunctioned, causing all most of all of the medication to leak on the patient's arm"); and clinical course details. Follow-up attempts are completed. No further information is expected.

Patient came by the pharmacy on 5/10/23 at 11:00 am to show me his arm. The injection site was red and swollen. It was warm and hard to the touch. The Axillary area was also swollen.

Primary care provider sent over both doxycycline and Augmentin for treatment today, 5/2/23. Patient reported that the rph who gave her the vaccine used the same alcohol swab to clean both of her arms instead of using a fresh one.

Left arm (deltoid muscle) red, swollen, and hot to the touch, onset 1 day after receiving the vaccine. Patient was evaluated at clinic by CRNP. Treatment- ice 20 minutes on 20 minutes off 3-5 times a day. Benadryl 25 mg 3 times a day and Tylenol as needed. Outcome-if no improvement, patient to follow up in the office in 3-5 days.

Site: Redness at Injection Site-Severe, Site: Swelling at Injection Site-Severe, Systemic: Allergic: Rash (specify: facial area, extremeties)-Severe, Systemic: Body Aches Generalized-Medium, Additional Details: Patient reported flu-like symptoms that developed with-in 24 hours of vaccine administration, on day 2 her arm "felt heavy" by day 3 rash developed that started at injection site and spread over the next days until she reported to pharmacy on 04/11/2023 in the afternoon. rash has spread on both side of injection site and down the arm to the wrist, rash was warm to the touch, skin not affected by rash on arm was not warm. Patient was advised to seeek medical attention at nearby urgent care.
