A female patient received BNT162b2 (BNT162B2), on 08Mar2021 as dose 1, single (Lot number: GN6206) for covid-19 immunisation; elasomeran (MODERNA COVID-19 VACCINE), on 22Apr2022 as dose 4 (booster), single (Lot number: 002M21A) for covid-19 immunisation. The patient's relevant medical history included: "Skin allergies to tapes" (unspecified if ongoing); "allergies to Latex" (unspecified if ongoing); "allergies to Steroid" (unspecified if ongoing); "allergies to Penicillin" (unspecified if ongoing); "allergies to Soap" (unspecified if ongoing). Medical Hx relating to mentioned AE and both Acute and Chronic illnesses at the time of vaccination reported as no. Other vaccines given within 1 month prior to Moderna COVID-19 vaccine (type, brand, lot, route, dose #) reported as no. Did AE cause patient to seek medical care (office visit, IJrgent care, ER, hospitalized) reported as no.
