Symptomtext

Patient states he is still experiencing pain at the injection site 1 month after he received the shot. There is no physical signs of any external or internal damage. No swelling, redness, or bumps at the skin around and near the injection site.

Rash Rash pruritic

Symptomtext

Clinic calls to report adverse reaction to Shingrix Vac he received on 15 JAN. States within 24 hours he broke out in all over body rash that resembled "chicken pox" that was very itchy for 3 days. Denies anaphylatic symptoms. He states today rash has resolved. Denies need to be seen. I did give him VAERS phone number and notified that I am obligated to make a report in their database. He stayed on phone with me while I filled out online form.
