Narrative: A 71yo male with PMH of coronary Bypass surgery, Chronic Periodontitis, Coronary Artery Disease, Hypertension, Hyperlipidemia and Adult Hydrocele Received Janssen Covid - 19 vaccine 0.5ml In the right Deltoid per policy and protocol, on March 20, 2021 No ADR was reported Per Physicians April 14, 2021 documentation, the patient was hospitalized for SOB and under Hospice Care @ Hospice Patient passed away on April of 2021 cause of death not specified

Hospitalized for anticoagulation initiation. Stable for discharge 4/29. Heparinoids used for treatment of clot.

chest pain, leg swelling Narrative: Pt reports having chest pain 2 days after receiving vaccine on 3/12/21 and lasting 2 days. Reports bilat lower legs swelling starting 7 days after vaccine and legs have remained swollen. Have not worsened but have not improved on 4/15/2021 post vaccine follow-up call. No pain or redness or soa. H/O PE in 2017. Nurse advised pt with bilat leg swelling and no improvement and h/o of PE to go to ED & to let ED know of jj vac. Pt instructed to call clinic for f/u once seen.

Headache

Symptomtext

Narrative: Pt received the vaccine on 3-15-21 and states that on 3-20-21 woke up with a "jack hammer" headache that lasted for 3 days and then a mild headache for a couple of days after this. States that she had to lie down due to the h/a. Denied that she had visual changes, difficulty thinking or any other symptoms. Headache lasted for a week total and has since resolved and deines any other symptoms at this time. Nurse informed pt that she would relay report to Doctor and advised pt to seek ER treatment if h/a or any other symptoms returned. PT aware and voiced understanding.
