Anxiety Blood pressure decreased Dizziness Syncope

Symptomtext

Vaccine received, patient felt dizzy and faint after vaccine received. Laid patient down on exam table. Patient stated started to feel better when laying down. She stated she was scared to get vaccine and had anxiety. After 5 minutes BP 130/79. Sitting up BP 108/48. She began to feel dizzy again and laid back down for another 5 minutes. Standing up BP 93/77 and patient stated felt better and could walk to car. Walked patient to car.

Employee had Hep B vaccination on 01/28/2021. On 02/01/21, he called the facility from home reporting that on 01/30/21, he noticed a red itching rash on his neck but thought it was due to razor burn/shaving. On 02/01/21, the itching had progressively gotten worse and he noticed the rash on his upper arms bilaterally and his neck. Employee was sent to Express Care for evaluation and treatment

Expired EngerixB administered; This case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 40-year-old patient who received HBV (Engerix B) (batch number LG5RN, expiry date 20th August 2022) for prophylaxis. On 29th August 2022, the patient received Engerix B. On 29th August 2022, unknown after receiving Engerix B, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. Additional Information: GSK Receipt Date: 29-AUG-2022 Reporter's Comments: Office manager reporting an expired dose of Engerix-B was administered on 29th August 2022. Dose expired on 20th August 2022. The reporter consented to follow up. AE report contains all details provided by HCP. Additional Supportive Information: The patient was administered with Engerix-B vaccine that was already expired on 20th August 2022.

Expired Engerix-B administered; This case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 49-year-old female patient who received HBV (Engerix B) (batch number LG5RN, expiry date 20th August 2022) for prophylaxis. On 25th August 2022, the patient received the 3rd dose of Engerix B. On 25th August 2022, unknown after receiving Engerix B, the patient experienced expired vaccine used. On an unknown date, the outcome of the expired vaccine used was unknown. Additional Information: GSK receipt date: 25-AUG-2022 Reporter's comment: Medical Assistant(MA) reported that the patient was administered a dose of Engerix-B on 25th August 2022 which had an expiration date of 20th August 2022. At the time of reporting, the dose has not been repeated. The Vaccine Administration Facility is the same as Primary Reporter. The reporter consented to follow-up. Additional Supportive Information: The patient received a dose of Engerix-B which was already expired, which led to expired vaccine used.

On 18th February 2022, the patient received Engerix B adult (intramuscular). On 18th February 2022, unknown after receiving Engerix B adult, the patient experienced overdose and adult product administered to child. On an unknown date, the outcome of the overdose and adult product administered to child were unknown. Additional Information: GSK Receipt Date: 21-APR-2022 Reporter's comment: The reporter was nurse from pediatric clinic and stated, the patient received an adult Engerix-B dose. No further information was known. The reporter consented to follow up. Additional Supportive Information: The pediatric patient received an adult Engerix-B dose, which led to adult product administered to child and overdose.
