Patient's wife reported the adverse event to me today 6/27/22 along with her husband. Patient received both Pfizer 2nd booster and Shingrix in left arm at approximately 4 pm 4/19/22. When he woke up in the morning 4/20/22 when walking from bedroom to the kitchen he fainted and hit his head. I was not dehydrated. They called 911 and was taken to the Emergency Room. The ER doctor did an EKG and xrays and they didn't find any other cause for the syncope. The MD said it was not a good idea to get shingrix and covid vaccines in the same day. The patient was discharged from the ER 3 hours later. The patient was a little tired the next couple of days. Did not have a concussion and no further problems reported.

Patient was helping husband make breakfast in morning (one day post-vaccine) and suffered a moment of unconsciousness resulting in the patient hitting her face and fracturing her jaw in 2 places. She was taken to the emergency department where she received stitches and prescriptions for an antibiotic and pain medication. The hospital could not find any other root cause of the fall and the only new factor in the patient's life was the vaccine. Hospital is unsure if this is what caused it but cannot rule it out.

Suspected vaccination failure; Shingles; burning throbbing pain in the right lower back radiating to the right abdomen; hypersensitivity of the skin in this area; This serious case was reported by a consumer and described the occurrence of vaccination failure in a 69-year-old female patient who received Herpes zoster (Shingles vaccine) (batch number L63B4, expiry date 08-APR-2023) for prophylaxis. Co-suspect products included Herpes zoster (Shingles vaccine) (batch number N3755) for prophylaxis. Concurrent medical conditions included diabetes, hypertension, hyperlipidemia, hypothyroidism and peripheral vascular disease. Concomitant products included metformin, valsartan, bisoprolol fumarate, rosuvastatin, levothyroxine, cilostazol (Pletal) and clopidogrel bisulfate (Plavix). On 26-JUN-2022, the patient received the 2nd dose of Shingles vaccine 50 ug. On 15-MAR-2022, the patient received the 1st dose of Shingles vaccine 50 ug. On 14-SEP-2023, 445 days after receiving Shingles vaccine and 548 days after receiving Shingles vaccine, the patient experienced vaccination failure (Verbatim: Suspected vaccination failure) (serious criteria GSK medically significant), shingles (Verbatim: Shingles), pain (Verbatim: burning throbbing pain in the right lower back radiating to the right abdomen) and hypersensitivity (Verbatim: hypersensitivity of the skin in this area). The patient was treated with methylprednisolone (Medrol) and valaciclovir hydrochloride (Valtrex). The outcome of the vaccination failure was unknown and the outcome of the shingles, pain and hypersensitivity were not resolved. It was unknown if the reporter considered the vaccination failure, shingles, pain and hypersensitivity to be related to Shingles vaccine and Shingles vaccine. It was unknown if the company considered the vaccination failure, shingles, pain and hypersensitivity to be related to Shingles vaccine and Shingles vaccine. Additional Information: GSK Receipt Date: 12-OCT-2023 This report was submitted via the online direct entry reporting system. The patient was not pregnant. The patient self-reported this case. The patient received other products concomitantly. The patient received Shingrix and had burning throbbing pain in the right lower back radiating to the right abdomen with hypersensitivity of the skin in this area and shingles. The patient had no rash. The symptoms were treated with Medrol dosepack and currently on Valtrex for almost a week. The symptoms began from 14-SEP-2023 and still ongoing. The symptoms were treated. This case was considered as suspected vaccination failure since the details regarding laboratory test confirming shingles were unknown at the time of reporting.

woke 6am on 3/11/22 with shivers, cold hands/feet, sore muscles throughout body, nausea, fever of 99.1. After rising, immediately started sweating, tunnel vision and threw up 3X. Dry heaves continued for 3-4 mins, along with extremely painful stomach cramps and back ache. Eventually subsided, ate 1/2 banana and water along with 3 ibuprofen. Slept approximately 2 hours. Woke with headache, but no fever. At 3pm, fever was 102.1, chills again. Eventually subsided to just fatigue, soreness, headache rest of day/eve. 3/12/22 - woke with red bump rash on neck, chest, stomach, groin, back. - Fever gone.

Both arms red and tender localized at injection site. Left arm with mild induration at injection site. Per patient redness and tenderness were resolving compared to site condition the first two days post vaccination. No concerns of skin infection at this time; per patient local and similar reactions happened before with other vaccines including prior doses of Pfizer covid19.

The day after receiving the Shingrix vaccine, symptoms of shingles in my right eye began to appear. Within less than a week Dr diagnosed it as shingles. She prescribed Valacyclovir, Prednisolone Ac 1% Eye Drop, Gabapentin, Ibuprofen, Acetaminophen. The incredibly intense pain lasted 6 weeks.

patient did not receive the full dose; This case was reported by a pharmacist via call center representative and described the occurrence of accidental underdose in a 65-year-old male patient who received Herpes zoster (Shingrix) (batch number N3755, expiry date 26th June 2023) for prophylaxis. On 7th April 2022, the patient received Shingrix (intramuscular) .5 ml. On 7th April 2022, unknown after receiving Shingrix, the patient experienced accidental underdose. On an unknown date, the outcome of the accidental underdose was unknown. Additional Information: GSK Receipt Date: 07-Apr-2022 Reporter's Comments: The reporter stated that while administering the Shingrix vaccine there was an issue with the syringe and the patient did not receive the full dose of Shingrix. The reorter did not have patient's Healthcare professionals information. The reporter did not consent to follow-up. Additional Supportive Information: The patient did not receive the full dose of Shingrix, which led to accidental underdose.

When technician was administering the dose needle retracted prematurely outside of the arm and patient did not receive the whole dose. contacted gsk/shingrix manufacturer and anything less than <0.45ml is and invalid dose and CDC recommends revaccination with an underrage the same day.

On 16th March 2022, unknown after receiving Shingrix, the patient experienced inappropriate dose of vaccine administered and inappropriate preparation of medication. On an unknown date, the outcome of the inappropriate dose of vaccine administered and inappropriate preparation of medication were unknown. Additional details were provided as follows: The Medical assistant reported that the patient received only the adjuvant of Shingrix, which led to inappropriate dose of vaccine administered and inappropriate preparation of medication.
