Fluarix vaccine was received in clinic on 9/15/21. Reportedly, it was discovered to be out of refrigeration around 9/20/2021 and was placed in the fridge at that time and the person didn't tell anyone what they did. I received the vaccine on 10/6/2021. Someone in the clinic told us in Dec 2021 that the vaccine had been left out of the refrigerator without knowing that 7 of us in the pharmacy had received that vaccine.
