Child here with mother today for vaccinations. Received MMR, HPAC, and DHI in left anterior thigh. Received VZV, PCV13, and HIB in right anterior thigh. Immediately after receiving vaccines, noted local site reaction, erythematous flat rash, to right thigh. No other reactions noted at this time. Nurse manager notified. Mother opted to take child to local emergency room via personal vehicle. Attempted to contact mother twice x 2 attempts.

Symptomtext

Developed red lump on outer left thigh 2/11/21. Mom reports warmth and pain to palpation. On exam, there is a dime-sized firm lesion on outer upper left thigh slightly warm to touch. Patient cries with palpation. No bleeding or drainage. No evidence of abscess. Patient is treated for possible cellulitis with Keflex times 7 days. Advised to apply warm compress 3-4 times daily. Plan to f/u in 3-5 days for recheck.

Crying Retching Screaming Secretion discharge Vomiting

Adelynn tensed up her legs and began to frown as I was cleaning her thigh with alcohol . After the first injection, she began to scream and cry. I injected the other two and applied band aids. As her mother was comforting her, she began to gag and expelled sme mucus, then threw up. The vomit was thick and a creamy color, with some chunks of fruit (oranges) in it. There was also clear mucus. She threw up twice more over the next five minutes. She calmed down and stopped crying for about five minutes, then started crying again. Her father took her out of the room. When he returned with her 20 minutes later, he said she had stopped crying almost immediately and was fine. I observed her walking, talking, and not in any apparent distress. Her mother said she had not ever thrown up after previous vaccinations, but that at home she sometimes gets herself worked up when she is upset and does throw up.

Symptomtext

At 12:05PM client states he started to feel dizzy and nauseated. O2 was given via facemask at 2L while being fanned. Vitals where taken and are as follows BP:97/48, BPM:55 and O2:99. Client was given sugar in a form of a cracker and also requested to have a sip of water. After fifteen minuets client states he felt well and wanted O2 taken off. Client was also help off examination table to bench. Client had a good gait and was able to walk by himself. After five minutes of being on bench last set of vitals where taken and are as follows BP:130/86, BPM:93 and O2:97. Client left clinic without any s/s of distress.

Patient's mom called 01/05/2021 @1106 to report that "she developed fever Sunday 12/27/20 of 102 gave Tylenol for fever and would only go down to 101, took her to the Dr. (Pediatrician) 12/28/20 and then continued Tylenol to treat fever" "Thursday 12/31 fever decreased to 100 and stayed down after that" "and then last night (01/04/21) she got a rash "pinkish dots not really raised and were on her upper torso" Mom also reports, "she has decreased appetite and drinking in the last week" Mom denies any history of a vaccine reaction in the past. No complaints of nausea, vomiting or diarrhea; Mom also reports, "I called her doctor today but haven't heard back yet" 01/05/21 @1303 called Pediatrician Dr. regarding mom's concern "could she have reaction to measle vaccine?" LVM for the "Dr.'s nurse" @ 1330 Dr. Office nurse reports, "viral illness 12/28" they will follow up with mom.

patient received a pediatric dose / on July 7th, 2021; patient received a pediatric dose / on July 7th, 2021; patient received a pediatric dose / on July 7th, 2021; This case was reported by a other health professional via call center representative and described the occurrence of inappropriate age at vaccine administration in a 25-year-old female patient who received HAV (Havrix 360 Junior) (batch number N592C, expiry date 12th June 2022) for prophylaxis. On 7th July 2021, the patient received the 1st dose of Havrix 360 Junior. On 7th July 2021, unknown after receiving Havrix 360 Junior, the patient experienced inappropriate age at vaccine administration, wrong vaccine administered and underdose. On an unknown date, the outcome of the inappropriate age at vaccine administration, wrong vaccine administered and underdose were unknown. Additional details were reported as follows: The adult female patient received pediatric dose of Havrix. Health care professional called and asked if the patient can receive another pediatric dose of havrix to complete adult dose. The patient didn't received till second dose of havrix. Consented to follow up.

Administered the vaccines that were stored outside of normal range; This case was reported by a other health professional via call center representative and described the occurrence of incorrect storage of drug in a patient who received HAV (Havrix) (batch number N592C, expiry date 12th June 2022) for prophylaxis. On 2nd April 2021, the patient received Havrix. On an unknown date, unknown after receiving Havrix, the patient experienced incorrect storage of drug. On an unknown date, the outcome of the incorrect storage of drug was unknown. Additional details were provided as follows: The reporter was a clinical supervisor. The age at vaccination was not reported. The patient received Havrix vaccine which was stored at minimum 29 Fahrenheit temperatures for 48 hours, which led to incorrect storage of drug. The reporter stated that the vaccine was inadvertently administered which was stored outside of normal range and needed to be discarded. The reporter was unable to provide additional vaccine and patient details. The reporter did not consent to follow up. product storage error; This spontaneous report was received from a licensed practical nurse regarding a 1 year old male patient. No information was provided regarding medical history, concurrent conditions, or concomitant medications. On 13-DEC-2019, the patient received improperly stored doses of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live(M-M-R II) lot # S037501, expiration date of 12-NOV-2021, subcutaneous in the right thigh , varicella virus vaccine live (oka/merck)(VARIVAX) lot # T017686 with expiration date of 21-MAY-2022, subcutaneous in the left thigh, hepatitis a virus vaccine inactivated(HAVRIX) lot # N592C with expiration date of 12-JUN-2022, intramuscular in the right vastus lateralis and pneumococcal 13v conj vaccine (crm197)(PREVNAR 13) lot # D57723 with expiration date of 30-SEP-2022 intramuscular in the left vastus lateralis, all for prophylaxis The vaccines experienced temperature excursions with a total of 29.5h.
