Patient was given the 1st dose of shingrix at his doctor's office on 2/1/19, then came to our pharmacy and received a 2nd dose on 2/28/19. Patient then went back to doctor office in the 2-6 month window for his 2nd dose with office, then came back to us on 6/29/19 for 2nd dose with us . This was not discovered that the patient had 4 doses of shingrix in a 4 month period until 4/14/21 at a office visit. No adverse events were reported.
