A 55-year-old non-pregnant female patient received BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Lot number: NDC number: NDC:59267-1000-01(as reported)), via an unspecified route of administration in the left arm on 19Mar2021 at 16:00 (at the age of 55-years-old) as a single dose for COVID-19 immunisation. Medical history included migraines and seasonal allergies. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. The patient was not allergic to medication, food and other products. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. Concomitant medications within two weeks of vaccination included Vitamin D and C. The patient experienced fatigue, bad headache (migraine type), nausea, vomiting and mild soreness at injection site on 20Mar2021 at 05:00. The events did not result in doctor or other healthcare professional office/clinic visit, and emergency room/department or urgent care The patient did not receive treatment for reported events. The clinical outcome of fatigue, bad headache (migraine type), nausea, vomiting and mild soreness at injection site was resolved on an unknown date in Mar2021. No follow-up attempts are needed. The patient is a 53yo F with history of asthma, non-epileptic seizures who on 5/4/21 received the 1st Pfizer vaccine of her vaccination series. Immediately afterward, she developed full-body shaking and SOB. She was immediately sent to the ER where she was found to have stridor but without angioedema, hypoxia, urticaria, vomiting, hypotension, or other signs of anaphylaxis. During her stay in the ER, I oversaw her care and her symptoms completely resolved after IM Epi, benadryl, famotidine, albuterol/iptratropium, steroids, and magnesium. After an observation period in the ED, she had no recurrence of symptoms and was ultimately discharged.

Medical history included hypertension and high cholesterol. The patient had no known allergies to medications, food, or other products. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient did not receive any other vaccines within four weeks prior to the vaccination. Concomitant medications included amlodipine (MANUFACTURER UNKNOWN) 2.5 mg, atorvastatin (MANUFACTURER UNKNOWN) 10 mg and olopatadine (MANUFACTURER UNKNOWN); all for an unknown indication from an unknown date and unknown if ongoing. On 05Mar2021 at 20:00, the patient experienced benign paroxysmal positional vertigo (BPPV), dizziness only when lying down and also elevated blood pressure. It was reported that, there was no other side effects at the time of reporting. On 05Mar2021 at 20:00, the patient underwent blood pressure test and the result was elevated blood pressure of 158/85 (unspecified units). I started with severe chills and fever of 101.4, and nausea during the night about 15 hours after the vaccination. Within a few hours I was so weak I couldn't get out of bed by myself. When my husband helped me up, I started to black out. He got me to the bathroom and I couldn't sit up on the toilet by myself and became so nauseated I got the dry heaves. My husband was holding me up and trying to keep me from going completely unconscious. He gave me a wet washcloth to put on my face, but I was to weak to hold it myself. I became drenched in sweat. Once my husband got me back to bed, I fell asleep and dry heaves stopped. I took Tylenol for the fever and that reduced my fever. I slept through that day and night and the next day I felt fatigued and nauseated, but not so weak and the fever was reduced significantly.

Approximately 45 minutes after receiving the vaccine in my right arm, I experienced a sharp headache in my left temple that was brief (under 10 seconds) followed by a drop in energy - similar to going limp but I caught it before it reached that point and "pulled myself together" as I was driving on my way home. Once home, I felt "woozy" but not quite dizzy or weak for about 3 hours. I slept fine and the next day I felt like my body was "bus" and attempted to do a word search puzzle. Normally the words will jump out at me however, at first, it looked like just a square of letters and I had to work at finding the words in the puzzle. On the third day, I felt energized and it wasn't as difficult to do the puzzle however not as easy as it was before the vaccine. Over the course of the last month I felt as if my thinking was disorganized however my jobs do require a fair amount of multitasking so I'm sure that didn't help with my brain processing the vaccine at the same time. My thinking has gradually returned to normal and as of approximately 06/22/2021 is pretty much back to normal and organized and the words in the word search puzzle are again "jumping out at me". The only remaining change since the first dose is a feeling of an energy charge however I have been boosting my vitamins a bit in order to recover from the side effects of the first dose of the vaccine.

About nine days after the first Pfizer vaccine I noticed a pain in my left tooth where is was difficult to chew. I thought perhaps it was a cracked tooth or abscess but it spread throughout my mouth making it painful to chew any food. It seems that the nerves above the teeth were somehow infected or something. I had pain around my eyes and once in my left ear. I suspected Sinusitis or something but was not sure as I had never had this happen before. I had my dentist look at it. The metal tapping tool showed no cracks. The xrays showed no tooth problems, nor any sign of sinus inflammation. I reported it to me doctor and he told me to report it on VAERS as a possible side effect of the vaccine. It was severe for about two weeks and has since returned to normal.

At the above mentioned time I started getting a headache and started getting dizzy. Both lasted until about 7:30pm. I had to lay down the entire time. To light headed/dizzy to stand. I took a prescribed tylenol , which I had for knee pains. Afterwards I felt weak, but fine.

ADVERSE EVENTS: 9/7/21: SUDDEN SENSORINEURAL HEARING LOSS IN LEFT EAR WITHIN 24 HOURS FO RECEIVING 3RD PFIZER VACCINE; TINNITUS IN LEFT EAR - WOKE UP IN MORNING WITH RINGING IN EAR & HYPER SENSITIVITY TO SOUNDS; BY 5:00 THAT EVENING I WAS TOTALLY DEAF IN MY LEFT EAR AND RINGING CONTINUED. 9/7/21 On-line appointment with Urgent Care, prescribed prednisone, 40mg. 9/9/21 Appointment with Dr. (Otolarynglogist); Diagnosed with Sudden Sensorineural Hearing Loss and Tinnitus, left ear; prescribed oral prednisone, 60 mg. (see Oral Prednisone schedule below) and administered 1st Intratympanic steroid injection. (See attached report) 9/20/21 Appointment with MD (Otolaryngolist), Administered 2nd intratympanic steroid injection. 9/22/21 Appointment with MD (otolaryngolist, for 2nd opinion. Assessment: 1. Sensorineural hearing loss (SNHL) in left ear with unrestricted hearing in right ear. 2. Tinnitus of left ear 3. Sudden hearing loss, left ear. Recommended I try Hyperbaric Oxygen Therapy. 9/24/21 Begin Hyperbaric Oxygen Therapy (HBOT) at facility (see schedule of treatments below) 9/29/21 Appointment with MD (Otolaryngolist). Noted 15 dB improvement in hearing in the low frequencies with some work recognition. Administered 3rd Intratympanic steroid injection and recommended continuing with HBOT. 10/4/21 Appointment with MD (Otolaryngolist), for 3rd opinion. Confirmed SSNHL in left ear and based on severity of hearing loss, did not think I would regain hearing. I scheduled appointment for further evaluation in 2 months. 10/6/21 Appointment with MD (Otolaryngolist) . Noted no improvement in pure ton hearing. Still a 53dB pure-tone average, but word recognition is up to 64% (but this was with live, rather than recorded, word testing). Recommended continuing HBOT. 10/25/21 Appointment with MD (Otolaryngolist). Noted that my hearing had plateaued. Recommended that I be evaluated for hearing aid and trial period or cochlea21 40 mg r implant, if necessary. Complete Vitreous Detachment(VD) in right eye Transient flashes of light and gray "floaters" began appearing 11/09/2021. Emergency eye exam completed 11/12/2021. I asked my doctor the potential cause, explaining my friend received the very same booster shot from the very same pharmacy on 11/05/2021 and she began experiencing same health side effects 11/06/2021. He said no official studies have been done. But he noticed an notable increase in calls from his patients with the same symptoms beginning late 2020 after they received their COVID Vaccine I believe this potential adverse side effect should be noted and communicated to the public. In addition to the stress and cost of an emergency eye exam, I remain extremely anxious about the full loss of sight in my right eye if further retina damage occurs. I have a follow-up appointment in 6 weeks and have been instructed to monitor closely for additional symptoms.

Pressure in center area of chest front and back at about 0400 on Oct 30. This was about 13 hours after the booster shot was received. It was a pressure of about 5 on a scale of 10. The pressure lasted about 2 hours and then dissipated. After the first episode the pressure was reduced to about a 3 on a scale of 10 and come and goes and last about 2 hours. I have been taking Ibuprofen.

My ankles swelled up a day later on March 7th nd they are still swelled up now both ankles and up into my calves. Skin is so tight. Have had the following done on April 12th - Heart test along with Kidney and Liver test which were all good. Also had a blood test and for 73 years old I hd a perfect blood test with cholesterol number at 150 and my HDH and LDL and triglycerides were all 2 to 3 times better than recommended. 12-weeks later my ankles and calves are still swelled up. On May 25th I had an Ultra Sound on both legs from the groin down and it came back with NO blood clots.

My doctor waas going through a process of elimination. I hd a perfect blood test. Last May 25th I had an Ultra Sound on both legs for blood clots and that came back good.

I started to notice significant hair loss about a week after my first dose of my Pfizer Covid 19 shot. While I shampoo my hair and then combing it, I was losing a lot of head hair. Normally, I do lose some hair during this process. However, after my vaccine I noticed much more hair loss. As of this date, I continue to lose more than normal head hair when shampooing.
