suspected vaccination failure; Patient got HZ after getting shingrix series / Rash started / 1 month post his second dose / increased and worsened 2 months post second dose; Rash; This case was reported by a pharmacist via call center representative and described the occurrence of suspected vaccination failure in a 67-year-old male patient who received Herpes zoster (Shingrix) (batch number FY5H2, expiry date 25th May 2022) for prophylaxis. Co-suspect products included Herpes zoster (Shingrix) (batch number NE49X, expiry date 23rd August 2022) for prophylaxis. The patient's past medical history included herpes zoster. On 9th October 2020, the patient received the 1st dose of Shingrix. On an unknown date, the patient received the 2nd dose of Shingrix. On an unknown date, less than 5 months after receiving Shingrix and 1 month after receiving Shingrix, the patient experienced vaccination failure (serious criteria GSK medically significant), herpes zoster and rash. On an unknown date, the outcome of the vaccination failure, herpes zoster and rash were unknown. It was unknown if the reporter considered the vaccination failure, herpes zoster and rash to be related to Shingrix and Shingrix. Additional details were reported as follows: The patient had previous history of Herpes zoster prior to receiving Shingrix series. The patient experienced Herpes zoster after getting Shingrix series. The patient reported no immediate side effects but stated a rash started to form 1 month post his second dose. The rash then increased and worsened 2 months post second dose. This case was considered to be a suspected vaccination failure case, as the details regarding laboratory confirmation for shingles was unknown. The reporter consented to follow up.

Sx's 2 hours after injection: Left shoulder pain, headache, nausea, Left arm and neck pain. Patient had taken Advil Tylenol for pain. Patient had Telehealth with PCP on 12/16/21 since sx's were not better. Rx: Vicodin and Medrol Dose pack.

experienced a rash in her neck then in her chest and belly / new areas; This case was reported by a physician via call center representative and described the occurrence of rash zosteriform in a male patient who received Herpes zoster (Shingrix) (batch number NE49X, expiry date 23rd August 2022) for prophylaxis. On 4th March 2021, the patient received the 1st dose of Shingrix. In March 2021, 72 hrs after receiving Shingrix, the patient experienced rash zosteriform. On an unknown date, the outcome of the rash zosteriform was not recovered/not resolved. It was unknown if the reporter considered the rash zosteriform to be related to Shingrix. Additional details were provided as follows: The reporter was patient's spouse. The patient received 1st dose of Shingrix and experienced rash on back, chest and belly on 7th Mar 2021. The patient stated that, the rash was ongoing, getting in new areas and looks like shingles. No further information reported. The reporter consented to follow up

One hour later my left arm started swelling- painful to move and lift. Four hours later I had flu like symptoms- total body aches, sweating. Rashes and hives began developing on the left arm and through the night, spread throughout the body. I remained bedridden with body aches and hives for 4 days. Saturday Dr. told me to take Benedryl. Tuesday I was given prednisone to take. Hives continued so I was reissued prednisone 2/15/21. I saw another Dr. on 2/19/21 who prescribed Montelukast 10mg. and Zyrtec. As long as I took the meds the hives subsided. When meds wore off, the hives returned. I saw and allergist Dr. on 2/22/21 who told me to take a double of Zyrtec and Allegra. Today is 2/25/21 and the hive are still returning when the meds wear off.

After vaccination the patient said his arm was sore. And now sometimes he gets a sharp pain at the injection site. The pain is not there all the time but it does wake him if he is sleeping or if he moves his arm a certain way.

Symptomtext

Three days After the second shot was given, a rash appeared from the injection site, up the arm toward the shoulder, then the rash ran down the patient's back towards his waistline. The rash was approximately 3 inches wide at places on the patinet's back.

received 3 doses of Shingrix; This case was reported by a pharmacist via call center representative and described the occurrence of extra dose administered in a 77-year-old male patient who received Herpes zoster (Shingrix) (batch number NE49X, expiry date 23rd August 2022) for prophylaxis. Previously administered products included Shingrix (received 1st dose on 15th May 2018 with lot X3M27 and expiry date 29th August 2020) and Shingrix (received 2nd dose on 14th July 2018 with lot B9739 and expiry date 22nd November 2020). On 29th November 2021, the patient received the 3rd dose of Shingrix. On 29th November 2021, unknown after receiving Shingrix, the patient experienced extra dose administered. On an unknown date, the outcome of the extra dose administered was unknown. Additional Information: GSK receipt date: 03-May-2022 Reporter's comment: Pharmacist reported that he/she had a patient that received 3 doses of Shingrix over a 3 year time period. Consented to follow-up. Additional Supportive Information: The patient received 3 doses of Shingrix, which led to extra dose administered.

was administered a dose of SHINGRIX VAC / that had been stored in a freezer /had no symptoms; This case was reported by a other health professional via call center representative and described the occurrence of incorrect storage of drug in a female patient who received Herpes zoster (Shingrix) (batch number NE49X, expiry date 23rd August 2022) for prophylaxis. On 29th April 2021, the patient received the 1st dose of Shingrix (intramuscular) .5 ml. On an unknown date, unknown after receiving Shingrix, the patient experienced incorrect storage of drug. On an unknown date, the outcome of the incorrect storage of drug was unknown. Additional case details were reported as follows: The health care professional (Medical Assistant) reported On 29th April 2021 the patient was administered a dose of Shingrix vaccine 1D 50MCG/O.5ML that had been stored in a freezer which led to incorrect storage of drug. The reporter contacted the patient and the patient had no symptoms, Product Storage Error was resolved. The reporter consented to follow up. The Vaccine Administration Facility was the same as Primary Reporter. The Medical Assistant that administered the vaccine to the patient. This adverse event has been previously reported to the Vaccine Adverse Event Reporting System (VAERS) database by the medical clinic.
