Symptomtext

Pt was here for a WCC visit. Vaccines were administrated at 10:20am. Not long after, pt fainted for about 10-15 seconds. Medical assistant picked baby off the table and patted her back. Patient then became alert and started crying. Juice given to patient and provider was called. MD came to the patient and stated that he thinks she had a Vasovagal syncope. Noted large rash around both injection sites. Patient was then monitored for about 15-20 min and left acting normal. Mother was here with patient. I have reported to VAERS

Symptomtext

Patient was vaccinated in clinic and waited with mother for 15 minutes, no adverse reactions were noted. Patient's mother came back to the clinic one week later for a separate appointment and stated that patient had redness, swelling and a bump at the injection site on patient's left arm the day following the injection. Patient did not have a fever. Mother gave patient heat pack, ice pack and Tylenol on Thursday. Patient's mother gave Benadryl on Friday (10/13/23.) On 10/18/23, patient still had a bump at the injection site but no redness or swelling.

Accidental overdose

Symptomtext

patient received 2 doses of IPV at the same time; This non-serious case was reported by a other health professional via call center representative and described the occurrence of accidental overdose in a 6-month-old male patient who received DTPa-HBV-IPV (Pediarix) (batch number NL35L, expiry date 04-APR-2025) for prophylaxis. Co-suspect products included POLIO VACCINE (IPOL) for prophylaxis. Previously administered products included IPOL (1st dose received on an unknown date) and IPOL (2nd dose received on an unknown date). Concomitant products included DIPHTHERIA VACCINE TOXOID + PERTUSSIS VACCINE ACELLULAR + TETANUS VACCINE TOXOID (INFANRIX), HAEMOPHILUS B CONJUGATE VACCINE (MENINGOCOCCAL) (PEDVAXHIB) and HEPATITIS B VACCINE. On 10-JUL-2023, the patient received Pediarix and IPOL. On 10-JUL-2023, an unknown time after receiving Pediarix and an unknown time after receiving INFANRIX, the patient experienced accidental overdose (Verbatim: patient received 2 doses of IPV at the same time). The outcome of the accidental overdose was unknown. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 10-JUL-2023 The health care professional (HCP) called in to report the administration by mistake of Pediarix to a patient who received the same day Ipol, Infanrix, and PedvaxHib. The reporter mentioned that with Pediarix the patient received the 4th dose of HepB. The patient received the 5th Dtap (Infanrix) and because of the maladministration of Pediarix, the patient received 2 doses of IPV at the same time. The HCP asked for potential adverse reaction following vaccination. During the call, permission to follow up was not requested. The patient received by mistake Pediarix and Ipol on the same day, which led to accidental overdose.
