At time of injection on 12/20 pt immediately noted paresthesias in ulnar distribution of L arm, the next day still had L upper extremety symptoms and she felt myalgias and bilateral leg weakness.

Chills Exercise tolerance decreased Fatigue Headache Hypersomnia Pyrexia

Symptomtext

Roughly 11 hrs after injection woke up with chills, fever, headache. took Tylonol. Symptoms improved but persisted for 4 hours. Headache for 24 hours even with continued use of tylonol. Extreme fatigue--slept 18 hr first night, awake 4 hours, back to sleep for 12, awake 12, slept 10 hrs. Fatigue continued for 5 days overall--sleeping 10-11 hours a night and unable to exercise besides short walks.

RECEIVE THE ADJUCVANT ONLY; RECEIVE THE ADJUCVANT ONLY; This case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 53-year-old male patient who received Herpes zoster (Shingrix) (batch number NR2X5, expiry date 24th November 2023) for prophylaxis. On 7th November 2022, the patient received the 2nd dose of Shingrix. On 7th November 2022, unknown after receiving Shingrix, the patient experienced inappropriate preparation of medication and inappropriate dose of vaccine administered. On an unknown date, the outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were unknown. Additional Information: GSK receipt date: 16-Nov-2022 Reporter's comment: The information for the adverse event was given and medical information was relayed about recommendations made by the vaccine coalition. The vaccine administration facility was the same as primary reporter. The reporter consented to follow-up. This case was for 3 of 3 linked patients, reported by same reporter.
