Patient had a shingrix immunization on 5/30/2021. On the next morning at home (5/31/21), stated that he felt weak while in the kitchen, passed out and fell to the floor. The patient did not describe any injuries from the incident and said that he just felt weak and fatigued afterwards. Patient chose to rest the day. Patient does not have note of his blood pressure at the time and no medications on file to assess contributing factors. Today, he states he is feeling better. Advised to follow up with provider as well. Left message for patient to get information regarding provider as to notify primary regarding incident

Used to have psoriasis / started with an break out all over her body(arms, back, legs); This case was reported by a pharmacist via call center representative and described the occurrence of psoriasis aggravated in a 53-year-old female patient who received Herpes zoster (Shingrix) (batch number NR55Z, expiry date 22nd October 2022) for prophylaxis. Concurrent medical conditions included psoriasis. On 15th May 2021, the patient received the 1st dose of Shingrix. In May 2021, 2 weeks after receiving Shingrix, the patient experienced psoriasis aggravated. On an unknown date, the outcome of the psoriasis aggravated was unknown. It was unknown if the reporter considered the psoriasis aggravated to be related to Shingrix. Additional details were provided as follows: The reporter was looking for information about Shingrix common side effects after 1st dose because the patient that used to have psoriasis received Shingrix 1st dose and two weeks later started with a breakout all over her body (arms, back, legs). The reporter consented to follow up by phone.

She felt dizziness and was unable to walk within 15 mins of leaving. She went to a grocery store after leaving health department had to have two people walk her to her car. Sat in her car for an hour. Once she got home she was able to walk into the house. Has laid around all afternoon and has felt fatigue. Vomited yellowish bile once she got home. States she now at 3:10 pm, feels better but does feel tired with a headache.

Pt experienced usual SE initially - was very fatigued w/ fever of 101 for 1.5 x days, HA, chills - those then resolved. On 5/22 she began having word finding difficulty, difficulty processing, vision deficits and has overall regressed back to where she was in 7/20 (immediately after brain surgery). Pt feels that acutely worsened symptoms were due to recieving Shingrix vaccine.

Patient called the clinic to report that she was given a SHINGRIX vaccine at the pharmacy on Tuesday 5/11/2021 after her clinic visit here with the doctor and developed swelling, pain that is a 6, red blotches below the injection site, tenderness, and itching. Doctor recommended the patient take Benadryl 25 mg, 1 tab now and Ibuprofen 200 mg, 3 tabs q 8 hours as needed for pain. If she develops more swelling, fever, chills to go to urgent care or ER (5/14/21). The reported symptoms were consistent with localized reaction to vaccine. Clinic nurse followed up with patient on 5/15/21. The patient stated the symptoms had already resolved.
