Symptomtext

Patient presented for 5 vaccines for travel. Administered Spikevax and VAQTA intramuscularly into left arm, Fluzone Quad and Boostrix intramuscularly into right arm. After 4th shot, patient began to feel light headed and stated that they did not eat anything for the day. Patient's head began to nod off 10 seconds later and patient began to sway back. Patient slumped over and black-outed for 2 seconds and became alert right after. Pt monitored till better.

Dizziness Syncope Unresponsive to stimuli

Symptomtext

Vasovagal syncope post vaccination (Tdap, Hep A- 1, TY- IM). Pt responded well to Vx administration in clinic room prior to walking out to waiting area. In waiting area Pt felt lightheaded so I had her sit in a chair, then I got her a cold pack. She sat there for another minute and I coached her to take slow, deep breaths. She then went limp while seated in the chair and was briefly nonresponsive. I called for help and 2 people came to the waiting area to assist. When 1 arrived the pt was already recovering and she brought her a juice to drink. I stayed with the pt for several more minutes to ensure her stability and check HR and RR (both WNL). I had the pt stay in the waiting area another 15 minutes rechecking her several times before walking she and her husband out of the dept. The pt stated after the fact that she has had syncopal episodes previously with Vx administration. (Her screening Dx was checked no for serious vaccine reactions and when asked prior to Vx administration pt stated that there were no previous issues with Vx.)

Symptomtext

Gave patient four vaccines, two in his right arm and two in his left arm. While in room patient tolerated the vaccines well. I walked him to the reception area to schedule his next vaccines. He stated he needed to sit down for a minute. As soon as he sat down, he passed out and all color drained from his face, he was also profusely sweating. Within a few seconds his body straightened out and I thought he was going to start having a seizure. I had the front staff call for help. One MA got a cold compress, another the BP machine and the third went and got the provider. He kept going in and out of consciousness for about two minutes. His first BP reading was 98/54. He was then given some apple juice and peanut butter crackers. His BP was repeated and was 117/74. He remained in the office until about 8:15 am. at that time he stated he felt well enough to drive two minutes to his home. He hadn't ate or drank before the vaccines. He also stated he has passed three other times during vaccines or blood draws.

PATIENT'S WIFE CALLED AN 1 HOUR AFTER RECEIVING VACCINE CLAIMING HER HUSBAND EXPERIENCED AN ADVERSE EVENT FROM THE BOOSTRIX VACCINE. CLAIMED HE WAS VERY DIZZY,LIGHTHEADED AND HAD PINS AND NEEDLES IN HIS ARM AFTER GETTING THE VACCINE. AS OF THIS REPORT I HAVE YET TO SPEAK TO PATIENT HIMSELF. MESSAGE WAS LEFT FOR PATIENT TO HEAR INFORMATION FIRST HAND

Patient develoved 2 raised lumps on arm around injection sites. They were hot to touch but after 3 days seemed to be going away. One shot was given in rear deltoid muscle and the other was almost on back of patient's arm.

Pt was given TDAP Vaccine in left arm, he then received Covid Pfizer Vaccine ( Comirnaty) in right upper arm, we proceeded to administer his influenza vaccine (fluzone ) in the right upper arm, as the provider finished administering the last vaccine - which was the influenza vaccine, the patient stated, "Uh-Oh, my fingers are getting numb and I am getting light headed" The provider proceeded to have him lay down on the exam table. the patient became very lethargic, he never lost consciousness. He apologized for getting lightheaded and states he donates blood regularly and has become faint and lightheaded in the past when donating blood. We had him drink some water, we asked him if he had eaten - he ate about 2 hours before coming to get his vaccines. We watched the patient for approximately 20 minutes at 7:50 pm , we raised the head of the exam table , at 7:53 pm , he was walking around in the exam room and states he feels fine. Patient was escorted to his vehicle, gait steady, alert and oriented.

additional dose of Boostrix on 01Nov2023; This non-serious case was reported by a nurse via call center representative and described the occurrence of drug dose administration interval too short in a 15-year-old female patient who received DTPa (Reduced antigen) (Boostrix) (batch number NR5DX, expiry date 02-FEB-2026) for prophylaxis. Previously administered products included Menactra (On 27th July 2020, received a dose) and Boostrix (On 5th May 2023, received a dose, batch number: 34FR4, expiry date: 23-SEP-2024). Concomitant products included MENVEO. On 01-NOV-2023, the patient received Boostrix. On 01-NOV-2023, an unknown time after receiving Boostrix and an unknown time after receiving MENVEO, the patient experienced drug dose administration interval too short (Verbatim: additional dose of Boostrix on 01Nov2023). The outcome of the drug dose administration interval too short was unknown. Additional Information: GSK Receipt Date: 1-NOV-2023 A nurse reported a patient received a dose of Menactra on 27th July 2020 and a dose of Menveo 01st November 2023. The patient also received a dose of Boostrix on 05th May 2023 and an additional dose of Boostrix on 01st November 2023. The reporter consented to follow up. The patient received another dose of Boostrix earlier than recommended schedule, which led to shortening of vaccine schedule.
