Symptomtext

Around 10/15/2020 the employee noticed a limited range of motion due to pain in her left shoulder and arm. Also started experiencing numbness in the left arm starting at the deltoid and traveling down to her fingers. Employee states no pain when arm is not moved. Greatest pain occurs when she reaches across her body. The pain and numbness got worse over time. Currently the employee is unable to buckle her seat belt or reach across her body without increased pain.

Symptomtext

Child received vaccines on 12/11/2020, 10 days later mom notice red hard welty area on (L) thigh. Now on 12/13/2021 Family had bout of vomiting the child also had low grade fever. So on 12/21/2020 had red hot to touch area with 3 Little pimples where vaccines given 10 days ago. Area is swelled and red in area. Then on 12/23 PCP seen voiced its a rash typical with MMR/Varicella Lump from injection.
