On Day 10-11 after the vaccination, my husband started developing tingling in his legs and arms with increased numbness and weakness in his leg muscles. He had a hard time breathing. He was admitted to the ER at Hospital. He is diagnosed with Guillian Barre Syndrome. His arms and legs are near paralyzed. He has been in the ICU for 6 days and is on a respirator

Per Walk In Clinic's notes: Patient 89 year old female presents today to clinic with complaints of #Vaccine reaction - Located on right upper arm - Received shingles vaccine on 21 July 2021 and notes symptoms started a few hours later - Now has redness, painful lump and area warm to touch - Denies shortness of breath, chest tightness Assessment and Plan: (T50.Z95A) Vaccine reaction, initial encounter (primary encounter diagnosis) Plan: - Discussed symptoms are a reaction to recent shingles vaccine - Outlined area of concern for monitoring - Recommend supportive home care; OTC Tylenol not to exceed 3,000mg/24 hours and cold compresses to the area - Will have clinic RN submit VAERS report - Follow-up with PCP as needed for concerns Return precautions given for fever, chills, chest pain/tightness, shortness of breath, palpitations, abdominal pain, nausea/vomiting, numbness/tingling/weakness in extremities, symptoms that worsen or don't improve with treatment plan

Concomitant products included ZOSTER VACCINE RECOMBINANT, ADJUVANTED (NON-LIVE) (SHINGRIX). On 28th September 2021, the patient received the 2nd dose of Shingrix (intramuscular). On 28th September 2021, unknown after receiving Shingrix and SHINGRIX, the patient experienced overdose. On an unknown date, the outcome of the overdose was unknown. Additional details provided were as follows: The registered pharmacist stated that he/she on the previous night shift administered two doses of vaccine to a patient in unknown deltoid, which led to overdose. The registered pharmacist stated that both the doses were from the same lot. The registered pharmacist stated that one dose was administered in each deltoid respectively. Concomitant products included Herpes zoster (Shingrix). On 10th September 2021, the patient received the 2nd dose of Shingrix. On 10th September 2021, unknown after receiving Shingrix and an unknown time after receiving Shingrix, the patient experienced drug dose administration interval too short. On an unknown date, the outcome of the drug dose administration interval too short was unknown. Additional details were provided as follows: The pharmacist reported that, the patient (who was not immunocompromised) received his first dose of Shingrix on 19th August 2021. The patient received his/her second dose on 10th September 2021, which led to drug dose administration interval too short. No further information reported. The reporter did not consent to follow up.

On 8th September 2021, the patient received Shingrix. On 8th September 2021, unknown after receiving Shingrix, the patient experienced incomplete dose administered. On an unknown date, the outcome of the incomplete dose administered was unknown. Additional details were provided as follows: The pharmacist reported that, he administered Shingrix and the needle was defective and either the medication started coming back out or was not going in. The pharmacist did not know how much of the medication, if any, the patient received, which led to incomplete dose administered. The reporter consented to follow up via email.

Patient called the pharmacy after receiving these 2 vaccines to state he had already received 2 shingrix doses and we just administered a 3rd. Upon review it was discovered his 2nd shingrix dose on 11-17-20 was not put in our state reporting database WIR. Upon review of pharmacy records it was discovered patient had received Shingrix on 9-18-20, 11-17-20 and 08-10-21
