Symptomtext

About 5 minutes (425pm) after both vaccines and routine hemoglobin finger poke (POC) pt was seated on the table, turned pale and fainted. She was out for 10 minutes, and after waking was dizzy and light-headed, although some of her pallor had improved. We gave her water and had her continue to rest. When sitting up, she felt nauseated and had one episode of emesis. After that, parent brought her a snack and she felt better. It was then I learned she had slept in late and hadn't eaten or drank much before her appointment with me that day. She was released with her parents, and walked out of the office by herself.

Symptomtext

Patient initially presented with postvaccination symptoms (initial vaccine was on 11/22/21 was given by medical assistant) via message on 12/9/2021 experiencing soreness in her left shoulder joint. She has not had any previous flu reactions. She was surprised at the time of the reaction due to her significant pain. She had difficulty with deep joint pain and ADLs such as difficulty pulling her shirt off, putting her hair in a ponytail. At that time we recommended seeing a provider for ongoing joint pain however patient did not follow-up. She was then seen again on 5/23/2022 during her annual physical exam complaining of ongoing L shoulder pain since her flu shot the previous fall. Some of this has resolved with passive range of motion exercises however she continues to struggle with flexion of her left shoulder past 90 degrees and abduction of her left shoulder. Denies any changes in strength or sensation. No previous injuries, traumas or surgeries. Physical exam revealed: Inspection of the left shoulder reveals no deformity, asymmetry, bruising, swelling or erythema. Full range of motion of bilateral shoulders to flexion, extension, internal rotation, external rotation. Strength 5 out of 5 in bilateral upper extremities Sensation intact bilaterally in upper extremities. No tenderness to palpation of L shoulder POSITIVE empty can test in affected shoulder. POSITIVE drop arm test negative in affected shoulder. No pain elicited on external rotation against resistance. Lift off test negative bilaterally. Suspected etiology at that time related to history and physical exam was a supraspinatus dysfunction/ rotator cuff syndrome.- unclear link to her flu shot however it did present immediately following administration (2 weeks later). Recommendation at that time to proceed with physical therapy- referral was placed. Patient continue to have pain and has not been seen in our clinic since then.

Pt felt a tingling sensation on the left side, which then moved over to the right side. Pt had taken an Acetaminophen tablet. Pt was advised to take a Benadryl tablet, but was advised that if she was concerned, to visit a Dr or ER.

Mom noticed a quarter size bump on the child's leg. The bump was hard to touch. Mom noticed the bump 10/29/21. No treatment done at this time.

Symptomtext

Patient reported to our pharmacy today (10/26/2021) that she has been experiencing moderate to severe shoulder pain in the left arm, that started two days after she received flu immunization. She states that she had almost no symptoms at all for the first two days after immunization, but then shoulder stiffness and pain started. The pain is moderate to severe and patient does report limited range of motion and worsening pain with arm use. Patient also reports some radiating pain into the elbow joint. Patient had tried OTC medication, which do NOT provide much relief, but icing has helped. Vaccination took place on 10/19/201, symptoms started two days later and to date are not improving. I suggested patient should get it looked at by her physician, especially if symptoms are not improving or getting worse.

Symptomtext

Patient stated she was having heartburn and right armpit soreness/pain about 3 to 4 minutes after injection. She waited about 15 minutes and the heartburn went away but she is still having armpit pain. It is staying the same, not getting better or worse.

Symptomtext

The products reached negative 31.5 Fahrenheit for thirty-eight minutes with no AE; Initial information received on 21-Oct-2021 regarding an unsolicited valid non-serious case received from another health professional. This case involves an unknown age and unknown gender patient for whom it was reported "the INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QIV] reached negative 31.5 Fahrenheit for thirty eight minutes" (product storage error). The patient medical history, medical treatment(s), vaccination(s), concomitant medication, and family history were not provided. On an unknown date, the patient received a dose of suspect INFLUENZA QUADRIVAL A-B VACCINE [prefilled, lot UT7383NA and expiry:30-Jun-2022] via an unknown route in unknown administration site for prophylactic vaccination. It was reported "the reason for excursion is unknown". It was a case actual medication error due to product storage error [Latency: on the same day]. At time of reporting, the outcome was unknown for the event. At the time of report, no adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
