Symptomtext

patient had a vasovagal reaction just after the covid vaccine was injected. Flu was administered just a moment before. Mom reports he has had this same response before with a needle stick (finger pricks for blood as well as vaccines) due to his fear of needles. He had decreased/tunnel vision, some belching, paleness, sweating. We helped him to lay on the floor, elevated his feet, fanned him off, applied cool water to his neck and he improved quickly. Kept for further observation for 20-30 min longer (had already been with him for that same amount of time) and he continued to feel good/ normal.

Symptomtext

Patient less than one minute after receiving vaccine started going faint then demonstrated what seemed to be a grand mal seizure. Parents were in the vaccine room with me. Patient went very pale and unresponsive. Brought patient to floor and raised legs into chair. Mother cradled her and splashed water on face. When patient came to, she was very lethargic. 911 was called. Fire department arrived and evaluated patient. Ambulance arrived and escorted patient with parents out of pharmacy. Patient has had no prior medical issues except fainting with blood draw.

I received an employee influenza vaccine on 09/15/2023. That night I began having a sore throat with difficulty swallowing and incontrollable drooling noted throughout the night. I woke up choking several times. Monday morning, I felt a little better and came to work . Monday 09/18/2023 one of the doctors at the clinic mentioned that my left eye looking abnormal but I thought nothing of it. I did mention that my eye has been burning really bad all day, but thought nothing of it because when I suffer with my migraines it effect that eye. I get really sharp pain, which I was not experience any signs of migraine on this day. Tuesday 09/19/2023 morning while performing my morning hygiene, I notice my face was pulling to the right side. I Went to urgent care that morning 09/19/2023 and was diagnosed with Bell's Palsy on left side and sinusitis. The doctor mention that my left eye is not closing all the way and i have numbness noted to left side of the face.

Immunization was given too high and into the shoulder joint space rather than the muscle. I could immediately feel the fluid flowing through my shoulder. Upon observation of the bandaid location, it was placed too high. I began having shoulder and bicep pain within an hour of the shot. It has now been 2 months and I am still having pain the in the shoulder and bicep/arm area

Patient alert and oriented to person, place, and time. At 10:40, patient denied dizziness. Patient stopped sweating. Patient stated, " I feel better". BP 139/89, P 56, R 20, equal and unlabored, Oxygen Saturation on room air 100%. No signs and symptoms of distress. Notified provider and patient cleared for discharge. Instructed to follow up with primary care physician. Instructed to report to ER or call EMS if any symptoms of distress occur. Patient and wife verbalized understanding.
