9/23/23 1 pm at a Tent event in the parking lot at the front of the store. Very busy event and did not just computers to double check ahead of the dose. The Tech and this writer (RPh) veiwed the PCQF form and missed that this 8 year old 95 Lb child was incorrectly given the 12 years and older Spikevax vaccine and flu shot. She was on the tail end of antibiotics for sinusitis and should note have been given a shot. at 6:30 pm we realized there error because or computer/system would not allow the claim to even select due to the age of the patient. at 6:35 PM this writer immediately called the grandmother to notify of this error. Patient had taken 480 mg of Acetaminophen at 5:30 pm and was sleeping. Instructed grandma to give her fluids, Ibuprofen 200 mg at 8:30 pm and every 6 hours for inflammation , aches and fever, alternating with tylenol 3 hours later if needed.Instructed her that if any difficulty breathing (bluish finger tips, lips, high fever or non responsive to go to the ER or call 911. On Monday 9/25/23 called Provider to report what happened. Called on 9/25/23 to Grandma to see how patient was doing. Grandma shared that on Sunday 9/24 she alternated the ibuprofen and acetaminophen and the child was dizzy twice when she took a shower. Her temperature was steady at 97.5 F and grandma's pulse oximeter showed 98-100%. Patient reported on Sunday 9/24/23 it hurt to breathe but that she was able to breathe through the night. Grandma listened to her breathe all night and said it didn't sound bad and she did not feel the need to contact medical help. On Monday 9/25/23 at 10 AM, grandma said patient was doing better and had not had any tylenol since 8 PM last night. It still hurt to breathe but that she was better. Patient got on the phone and described that it hurt less today and "hurt where a necklace would be." They were in the doctors office waiting room because Grandma just wanted to double check if she was doing OK. Shared that she could give another 200 mg of ibuprofen to help her feel comfortable or do what she thought was best. Shared that I would use this information to the CDC VAERS program in case this error happened to someone else. Grandma reported that the covid shot area had a 1 inche red itchy spot on her arm and the flu shot area had a 1 inch black and blue bruise.
