Patient then requested a trash bin and proceeded to vomit. Patient's condition returned to normal and he refused our offer to call for additional medical assistance.

On Tuesday I developed a raised itchy welt on my left deltoid that was approximately 2 inches long and 1/2 inch wide and was red and warm to the touch. By Wednesday it had spread to an area about 4 inches long and 2 inches wide. By Thursday the area from the top of my deltoid to my elbow was red and itchy and warm. By Friday it was past my elbow. By Saturday it was my entire arm from the deltoid to my wrist and in addition to the itchy red patches on Saturday night my arm was swelling so much that my elbow was no longer visible. By Sunday my arm was swollen to almost double the size. As I was on a cruise ship, I went to the medical center and was given 2.5% hydrocortizone cream and an injection of steroid.

On Saturday (3/26/22), patient stated she experienced tenderness near the injection site, fever, chills and body aches. By Monday, she felt pain upon raising her arm and it was painful to touch, hard and warm. So she made a visit with her physician and was diagnosed with cellulitis and prescribed her cephalexin. Patient stopped by the pharmacy on Thursday evening (3/31/22) to report and show me her rash that developed below the injection site.

patient called pharmacy around 5:15 PM and stated that her arm was hurting. She said it had been hurting for about 15 minutes and hurt from wrist to shoulder and had pain under her arm. She said she took some naproxen and was going to rest. She denied any symptoms of an allergic reaction. She said she would follow up with us/ her doctor tomorrow if she wasn't feeling any better.

On 27-OCT-2023, the patient received the 1st dose of Shingrix. On 27-OCT-2023, an unknown time after receiving Shingrix, the patient experienced expired vaccine used (Verbatim: patient was administered an expired dose of Shingrix). The outcome of the expired vaccine used was unknown. On 27th July 2022, the patient received the 1st dose of Shingrix. On 28th July 2022, 1 days after receiving Shingrix, the patient experienced swelling of legs. On an unknown date, the outcome of the swelling of legs was not recovered/not resolved. It was unknown if the reporter considered the swelling of legs to be related to Shingrix. Additional Information: GSK Receipt Date: 28-Jul-2022 Reporter's Comments: The pharmacist reported a patient received her dose of Shingrix late morning/early afternoon in left arm. The patient reported when she woke up she had left leg swelling. The Vaccine Administration Facility was the same as Primary Reporter. The follow consented to follow up via HCP email.

Patient accidently received third dose of Shingix vaccine. Patient presented to pharmacy and asked for Shingrix vaccine to be given, review of system did not find any history of Shingrix vaccines being given in past. Vaccine was processed successfully through insurance. First vaccine in series was given 3/26/22. Patient returned 5/4/22 for Moderna booster and asked when he would be due for his Shingrix booster and when he would need the vaccine series again, mentioned that this was his second time getting the series and he had received it once already in 2020 from another pharmacy. Review of Vaccine Registry found two doses of Shingrix given in 2020. Informed patient to not return for an additional dose of Shingrix and that third dose should not have been given.
