Symptomtext

Pt experienced mouth pain in the days following vaccination. Pt scheduled appt with dentist later that month. Dentist informed her that she had "pock marks" in a line on the roof of her mouth and the pain was originating from there.

patient received the Shingrix adjuvant portion only; patient received the Shingrix adjuvant portion only; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 59-year-old male patient who received Herpes zoster (Shingrix) (batch number ZP22N, expiry date 27-SEP-2025) for prophylaxis. On 08-NOV-2023, the patient received the 1st dose of Shingrix. On 08-NOV-2023, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: patient received the Shingrix adjuvant portion only) and inappropriate dose of vaccine administered (Verbatim: patient received the Shingrix adjuvant portion only). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were unknown. Additional Information: GSK Receipt date: 17-NOV-2023 The reporter reported that the patient received the Shingrix adjuvant portion only, which led to inappropriate preparation of medication and inappropriate dose of vaccine administered. The reporter consented to follow up.

Symptomtext

Patient received both vaccines and then reported about 2 hours later he lost vision in his left eye. He called pharmacy and we told him to go to the doctor to not take any chances and after investigating this could be a possible adverse reaction from the Shingrix Vaccine. Patient went to the hospital and they did test because at first they thought it could of been a stroke or diabetic complication. After test they realized that it was from the Shingrix vaccine attacking the optic nerve. The patient had to have surgery on the left eye. They had to do laser surgery on the left eye optical nerve. The patient was given antibiotic/steroid eye drop to use after surgery and reports back to the doctor in a week.
