Patient tested positive for COVID-19. Patient was fully vaccinated. He started feeling "different" about the middle of July. He was more fatigued, disoriented, and unstable. The following week, he developed cold symptoms which everyone assumed was just the common cold. Pt. daughter was sick the previous week with a common cold (took her 3yo to the doctor to confirm, but wasn't tested for Covid). After 7/26, his condition started to deteriorate more with fever and vomiting. By the evening of 07/30, he was having great difficulty breathing and so he was taken to the hospital. He was admitted for very low blood oxygen levels as well as Covid pneumonia. He has several underlying health conditions to include heart disease and obesity. Patient died due to COVID.

Having a minor respiratory issue where I could hear myself wheezing; Soreness around the site of the shot; Feeling tired; Muscle soreness; This is a spontaneous report from a contactable consumer, the patient. A 50-year-old male patient received the first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Lot number: 6205), via an unknown route of administration in the left arm on 16Mar2021 at 15:30 (at the age of 50-years-old) as a single dose for COVID-19 immunisation. Medical history of the patient included allergies. The patient did not have any allergies to medications, food, or other products. Prior to the vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. Concomitant medications included ZYRTEC. The patient did not receive any other vaccines within four weeks prior to the COVID-19 vaccination. On 16Mar2021 at 22:00, the patient experienced some soreness around the vaccination site of the shot, felt really tired along with muscle soreness. At 23:30, the patient experienced a minor respiratory issue, where patient could hear wheezing. The adverse event did not result in a visit to the doctors or other healthcare professional office/clinic visit, and emergency room/department or urgent care. The patient did not receive any treatment for the reported events. The clinical outcome of the vaccination site pain, tiredness and muscle soreness was resolved on 18Mar2021 (All the symptoms disappeared after 36 hours); while that of the wheezing was resolved on 17Mar2021 (this lasted about 1 hour). No follow-up attempts are possible. No further information is expected.

I am suffering from pain that started after getting my first Pfizer vaccination shot on 3/19/21. The pain has not gone away and so I went to see my doctor . My first Pfizer dose was given to me at Church: CDC Booking # 169333 03/19/21 @3:30pm. I have pain in my right arm since my vaccine and have limited motion in my arm before pain increases

SIRVA, bone bruise, tendinitis, frozen shoulder. Severe pain in left shoulder and arm, with shooting pain from left shoulder down to elbow, hand, and fingers on left hand, and numbness in fingertips, by the evening of the day of the vaccination. Not able to sleep on left side or carry moderately heavy items with left arm. Within days, unable to raise arm. About a week later, developed significant lower back pain, likely due to being propped up for GERD and uncomfortable arm sleeping position. On 4/1/21, began physical therapy for 3 weeks, which helped reduce symptoms somewhat. Continued PT exercises at home after that. In September, 2021, was finally able to lift left arm overhead. Varying levels of burning and shooting pain continue to persist in left arm and shoulder, which are aggravated by lifting moderately heavy items or sleeping on left side.

A 34-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE; Lot Number: 6205) via an unspecified route of administration in the left arm on 27Feb2021 at 12:00 (at the age of 34-year-old) as a single dose for COVID-19 immunisation. Patient Medical history include COVID-19 infection around mid Feb. The patient had not been tested for COVID-19 since the vaccination. The patients concomitant medications included pantaprozole (MANUFACTURER UNKNOWN), rosuvastatin (CRESTOR), simethicone (PHAZYME) and finasteride (MANUFACTURER UNKNOWN) all for unknown indications from unknown dates. The patient did not receive any other vaccine within four weeks prior to the vaccination. On 14Mar2021, at 03:00 after 2 weeks of vaccination patient experienced sleeplessness and tingling burning in arms. The patient did not receive any treatment for sleeplessness and tingling burning in arms. The clinical outcome of sleeplessness and tingling burning in arms was not resolved. No follow-up attempts are needed. No further information is expected.

Headache Migraine Nausea

Symptomtext

Approximately seven hours after receiving my vaccine, I began to experience a slight headache and slight nausea that turned into a migraine. I then rested, attempted to sleep, and used a cold wash cloth to alleviate symptomatic pain. The migraine eventually went away after about 14 hours.

Symptomtext

Vaccine given 3/17 @ 1150am. Started itching on hands and feeling SOB and CP 3/17 @ 1220. Brought to ER by EMS 3/17 @ 1255. ER did not note a rash but patient was scratching when arrived and examed no angioedema/wheezing/or strider. Was given po diphenhydramine 25mg x 1 and famotidine 20mg po x 1. Sx resolved.

An 82-year-old female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE; Lot Number: 6205), intramuscular, administered in left deltoid on 27Feb2021 (at the age of 82-years-old) as dose 2, single for COVID-19 immunization. Medical history ongoing hypothyroidism, depression and vitamin d deficiency. Concomitant medications included levothyroxine taken for hypothyroidism, paroxetine taken for depression and vitamin d taken for vitamin d deficiency. Historical vaccine included BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE; Lot Number: EN9581), intramuscular, administered in right deltoid on 06Feb2021 (at the age of 82-years-old) as dose 1, single for COVID-19 immunization after which the patient experienced chills, myalgia, pyrexia, pain in extremity, feeling cold, fatigue and arthropathy. The patient experienced chills, fever and muscles aches on 28Feb2021. The patient did not receive any treatment for the events. The outcome of the events was recovered on an unspecified date. COMMENTS: She had chills, fever, muscle aches with both Covid vaccine. Both times these symptoms started the late evening/ morning after shot and resolved 2 days later. She does not have any tingling symptoms. No follow-up attempts are required; information about lot/batch number cannot be obtained. No further information is expected.

1st shot- severe headache 3 days, 4th day eye muscles did not work together as if they were fighting each other. Incidence lasted 12 minutes. Have had this same headache and eyes not working several times. 2nd shot- extreme lethargy for 3 days. Repeatedly relived these same symptoms 2-3 days were lethargy incidence. 3 months after 2nd vaccine my heart rate became erratic for no apparent reason. Zero history of any heart issues. Main concern is the heart rate variation and the muscular Conflict in my eyes. Time of day is not relevant- it can happen any time.

Vomiting; nausea; I suffered from a severe episode of vertigo; dizziness; chills; aches; severe headache; This is a spontaneous report from a contactable consumer (patient). A 44-year-old female patient received BNT162B2 (Pfizer BioNTech), dose 2 via an unspecified route of administration, administered in Arm Left on 27Mar2021 08:45 (Lot Number: 8727) as single dose for COVID-19 immunisation. Medical history included oral allergy syndrome, and allergic to fruits like apples, pears, peaches etc and hazelnuts; and birth control. The patient was not pregnant. Concomitant medications included betamethasone for an unknown indication and ethinylestradiol, ferrous fumarate, norethisterone acetate (TRI LEGEST FE) for birth control. The patient previously took first single dose of BNT162B2 (Pfizer BioNTech) via unknown route of administration in left arm on 06Mar2021 09:15 (lot number: 6205). There were no other vaccines within four weeks. The patient reported outside of chills, aches, and severe headache 14 hours after 2nd injection (lasting 2 days) on 27Mar2021. Patient eventually felt fine - but then 7 days later on 03Apr2021 16:00 (4:00 PM), the following Saturday after the second dose, she suffered from a severe episode of vertigo/dizziness. She never had vertigo before in her life so it was odd. Also vomiting and nausea ensued. Patient landed in the ER. She had been stabilized and had been home since. Nausea and vomiting hasn't returned but the vertigo/dizziness persisted. Patient tried Epley and Foster moves but still dizzy at the time three days later (Tuesday 06Apr2021). Treatment for the events also included IV for rehydration, meclizine, and ondansetron. The patient did not have COVID prior vaccination. The patient was not COVID tested post vaccination. Chills, aches, and severe headache recovered on 29Mar2021, vomiting and nausea recovered on an unspecified date in Apr2021. The outcome of other events was not recovered.

Throwing up all day-started 2hrs after the vaccine.; This is a spontaneous report from a contactable non health care professional, patient. A 44-years old male patient received BNT162B2{COVID-19 MRNA VACCINE BIONTECH} [batch/lot number# 6205], via unspecified route in the left arm on 03Mar2021 for COVID-19 immunisation. Medical history included high blood pressure, dilated ventricle, ascending aorta dilation and left ventricular hypertrophy. Concomitant medication included lisinopril HCTZ (unknown manufacturer). The patient experienced "Throwing up all day" which started two hours after receiving the vaccine on 03Mar2021. The patient underwent Nasal Swab test twice, on 07Mar2021 and 13Mar2021, both reported as negative. The patient had not been diagnosed with COVID-19 before vaccination. The patient underwent PCR test for COVID-19 after the vaccination. The patient did not receive any other vaccines within 4 weeks prior to the COVID-19 vaccine. The patient did not have any allergies related to medication, food, or other products. Patient received anti-nausea medication as a therapeutic action for the event. The clinical outcome of the event was reported as recovered. No follow-up attempts are needed. No further information is expected.

Mild chest discomfort starting at 9:00am that lasted all day. Went to the ER and resting heart rate was 110. The following day heart rate dropped to 95 and the day after 80. No treatment was given and symptoms possibly have disappeared on their own.
