The day after vaccination EMS was called to the patients home EMS States: Pt had right sided gaze noted possibly for the past 2 days. upon EMS arrival pt had tonic clonic seizure activity x3, family denies history of seizures. 12 lead EKG showed afib RVR. Pt was stoke activated and transported to the hospital.

~a week and a half after Vaccine, I experienced 'room spinning' vertigo and dizziness at around 1am. I was sick for the next few hours, vomited for around the first hour after. Symptoms of dizziness for the next two days, then Vertigo/dizziness/vomiting again the following night. After around 3 more days, the symptoms subsided. I had my 2nd Moderna shot on 5/18/2021 (Lot 036C21A) and on 6/10/2021 the symptoms began again. After the first case, I saw online Urgent care and was referenced to an in-person visit to a primary care doctor. I was 'diagnosed' with BPPV, which I've never experienced symptoms of before. They ran me through several tests at the office, but by the time I got to the primary care doctor the symptoms had subsided again. I'm currently still experiencing a light dizziness 24/7 since 6/10/2021.

I was given directions to perform the Epley Maneuver, which doesn't seem to have much effect. I was given Ondansetron and Meclizine, of which I've only needed to consume one pill of each during the second vertigo phase. I vomited 10 minutes after, so that was that. I was also prescribed Flonase, which I assume is for the purposes of drying out the ear canal. However, the dizziness remains.

No adverse event; Patient received first dose on 22-Apr-2021 and second dose on 05-May-2022; This spontaneous case was reported by a patient and describes the occurrence of INAPPROPRIATE SCHEDULE OF PRODUCT ADMINISTRATION (Patient received first dose on 22-Apr-2021 and second dose on 05-May-2022) and NO ADVERSE EVENT (No adverse event) in a 59-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch nos. 050C21A and 002c1A) for COVID-19 prophylaxis. Concurrent medical conditions included Type 2 diabetes mellitus, BPH, Hypertension and Drug allergy (Unknown historical drug). On 22-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 05-May-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 05-May-2021, the patient experienced INAPPROPRIATE SCHEDULE OF PRODUCT ADMINISTRATION (Patient received first dose on 22-Apr-2021 and second dose on 05-May-2022). On an unknown date, the patient experienced NO ADVERSE EVENT (No adverse event). At the time of the report, INAPPROPRIATE SCHEDULE OF PRODUCT ADMINISTRATION (Patient received first dose on 22-Apr-2021 and second dose on 05-May-2022) and NO ADVERSE EVENT (No adverse event) outcome was unknown. Patient received third dose as Moderna booster on 04-Jan-2022, batch number (041JZ1A) and anatomical location was reported as left arm. Patient started to had Pfizer paxlovid drug (300-100 mg) on 03-Oct-2022 and stopped at 07-Oct-2022, Batch/lot number (1918538A) and indication used as (Treatment of COVID-19/COVID-19 Treatment) Patient was taking a lot of Contact for a list as a concomitant medication. Patient had body temperature and result were provided as 97.2 Fahrenheit and had BP with result as 140/79 On 11-Oct-2022 Patient had Paxlovid Rebound and tested positive (line extremely dark and immediate). It's back feeling cold like symptoms ? runny nose, sneezing, stifles, nasal congestion and run down, no headache, no achiness. Chest and breathing good, no coughing and outcome for this was reported as recovering/resolving. No treatment medication were provided. Device Date : 12-Oct-2022

Sciatica Systemic lupus erythematosus

Symptomtext

Lupus flare up; Sciatica Pain in the Hip; This spontaneous case was reported by a consumer (subsequently medically confirmed) and describes the occurrence of SYSTEMIC LUPUS ERYTHEMATOSUS (Lupus flare up) in a 58-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 002C1A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. The patient's past medical history included Sciatica and Arthritis. Concurrent medical conditions included Sjogren's syndrome and Lupus erythematosus systemic. On 20-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 21-Apr-2021, the patient experienced SYSTEMIC LUPUS ERYTHEMATOSUS (Lupus flare up) (seriousness criterion medically significant) and SCIATICA (Sciatica Pain in the Hip). On 27-Apr-2021, SYSTEMIC LUPUS ERYTHEMATOSUS (Lupus flare up) and SCIATICA (Sciatica Pain in the Hip) had resolved. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. No concomitant medications were provided. The patient visited the Emergency Room (Does not remember exact date but it happened after vaccination). Treatment medication reported includes Prednisone, Morphine and Phenergan.
