Patient received J&J x 1 on 9/29/21. Presented to ER on 6/6/22 and found to be covid positive. Was admitted. Workup included COVID-19 testing which was positive. He was mildly hypoxic. He has significant risk factors with COPD with continued nicotine addiction. Was started on remdesivir and dexamethasone as well supplemental oxygen per protocol, appropriate quarantine, and azithromycin and ceftriaxone. Was started on Breo Ellipta. Initially was in the ICU. Of note had 1 J and J vaccine but no booster. Over the next 72 hours he improved and was titrated off oxygen by discharge. Was counseled repeatedly regarding smoking cessation. In regards to syncope had echocardiogram which was unremarkable and was on telemetry without issues. Had no further episodes. Blood pressure medications were held tried his hospital stay but by the time of discharge blood pressure was starting to elevate. By the above date he was tolerating his usual activity and discharged home. Discharged on 6/10/22
