Symptomtext

Patient received a second dose of the flu vaccine 15-20 mins after of receiving the first dose on the same day. After the patient found out he had received a second dose he panicked, became dizzy, pale, and stated he felt tingling in his hands and feet. A blood pressure reading was taken and showed low blood pressure. He was sitting down throughout the whole event. Patient stayed in the counsel room with his parent, a pharmacist in charge and fellow pharmacist for observation for 10 to 15 minutes. Patient was offered 911 services but turned it down. Patient received water and a candy bar and recovered well enough to walk with his mom out of pharmacy to go home.

Symptomtext

Rash-pimple like to affected arm, in clusters started 2 days after injection. Spread to R chest and R back on affected side. Axillary lymph node swelling started approx 1 week after administration. Given antibiotics and prednisone to clear.

No distinct adverse event, but due to small bore needle use, and pressure exerted on syringe, needle became disconnected from syringe at time of delivery of flu vaccine and none or minimal dose entered patient intramuscularly. Clinical judgement exercised to administer additional dose.

Symptomtext

Approximately 5 minutes after second vaccine (Menveo), patient stated "I can't see anything" from the Immunization Waiting room. Patient appeared to be pale, blinking rapidly, pupils were dilated, and she was diaphoretic. Registered nursing staff contacted to assist, and several doctors came in to evaluate patient. Patient had low blood pressure, running in the 60's/40's. Sent to ER via stretcher.
