Symptomtext

Immediate injection site pain which continued for 2 weeks now. Exam indicates subacromial bursitis of right shoulder (vaccination arm). Treatment- subacromial corticosteroid injection 11/16/2023 given and home exercise packet.

Dizziness

Symptomtext

Patient received flu shot and was seated in chair waiting on grandmother who was in line. Passing customer stated the patient was having a seizure. When I saw patient she was not seizing but looked like she was going to pass out. This was approximately 5 minutes after the injection. We lowered patient to the floor and called EMS. Patient was lucid, communicated well, no additional signs of seizure. EMS arrived and loaded patient into ambulance.

Symptomtext

Patient presented with hives on face, neck, upper extremities and torso about 1.5 hours after receiving this flu shot. Patient was given Benadryl 50mg PO, instructed to utilize Hydrocortisone cream he has on-hand and to contact facility officers/health services staff immediately if any anaphylactic symptoms should occur.
