MOM WAS WITH PT INSIDE IMZ BOOTH. HE RECEIVED BOTH VACCINES. PT WAS NERVOUS AND MOM HELPED TO CALM HIM DOWN. HIS MOTHER ASKED HIM TO GO TO HIS FATHER. HIS FATHER WAS STANDING OUTSIDE THE IMZ BOOTHE. HE WALKED OVER TO HIS FATHER AFTER IMZ. I WAS THEN ABOUT TO GIVE HIS MOM AN IMZ ABOUT A MINUTE LATER WHEN I HEARD A COMMOTION OUTSIDE THE IMZ BOOTH. PT WAS LAYING ON THE FLOOR TRYING TO GET UP. HIS FATHER WAS KNEELING BESIDE HIM TELLING HIM TO STAY ON THE FLOOR. HIS FATHER SAID HE FAINTED. HE ASKED FOR SOMETHING WITH SUGAR. I GAVE THE FATHER A ROOT BEER SODA AND A PACKET OF YOUGURT. THE FATHER KEPT THE PT ON THE FLOOR FOR ABOUT 10 MINS. HE HAD HIM DRINK SOME OF THE SODA AND CONSUME SOME OF THE YUOGURT.

Symptomtext

Patient received Covid pediatric vaccination (Moderna) in left arm without reaction. Then received Flu vaccine in right arm and lost consciousness for approximately 2-5 seconds at approximately 4:45 pm. She was in seated position and was maintained in position by administering pharmacy technician. When patient regained consciousness, she reported more pain during flu vaccination which caused an emotional response. 911 was immediately called and EMS arrived at approximately 4:55 pm. Patient's blood pressure and heart rate was stated as "normal" by EMS and blood glucose 106 mg/dL. Patient's mother (who is an OB-GYN) declined further care.

Symptomtext

16 hours post vaccine child standing, went pale, coughed a "weird" cough, fell forward to the ground and was unconscious for few seconds, started shaking and drooling and was incontinent of urine. Once stood up he vomited in the hallway at school. EMT was called and examined child O2 was around 90, no fever noted. Parent took child to Pediatrician. Child temp was at 101.2, Dr had him drink Gatorade in which he vomited. Took about 30 minutes at Pediatricians prior to going home.

Symptomtext

patient fainted in aisle after having shot. Was alert while pharmacist spoke with him. Patient felt dizzy and had blurred vision, but was able to see clearly right away. Paramedics cleared him.

fever, body aches, headache, and vomiting began 11/15/23, day after vaccine with no symptoms before vaccine. I, her mother and a family nurse practitioner had similar symptoms but not the vomiting after my own Moderna booster

Patient was quickly tired after the shot. Went to bed without any supportive treatment at home and woke up with a 104.8 fever that took 30min to start to decrease with 160mg of feverall.

Symptomtext

Pt was seen in clinic to get influenza and COVID vaccine. MA drew up COVID vaccine from a SDV that was labeled as 0.25ml which was the appropriate dose for her age. COVID and flu vaccine was administered. Later, RN was drawing up 2 move COVID vaccines for 2 other pts under ages 12years and found that there was more that 0.25ml in SDV labeled as 0.25ml. RN called Moderna the manufacturer and representative did confirm that they are overfilling this SDV more that 0.25ml to ensure that when drawing up, pt gets the full amount of 0.25ml. Representative was not able to tell RN the exact amount that each vial contains but did start a case on this. Pts mom and dad contacted communicated that pt could have received more than 0.25ml appropriate dose. RN had mom and dad observe pt injection site and there were no issues noted at this time. RN educated mom and dad to come into clinic for any concerns to local site reaction such as redness, heat or hardening to site. Educated to take pt to ER for evaluation for any systemic reaction to include; trouble breathing, swelling to throat, tongue, eyes, trouble swallowing, hives etc. Mom and dad verbalized understanding. RN communicated that RN will f/u with mom and dad on Monday to check on pt status.
