10:20 AM Vaccine given - Pfizer 1st Dose 10:25 pt in BR coughing, heaving, vomiting, I went in to assist and he said his throat was itchy and he felt like he needed to cough and vomit and symptoms started right after vaccine Moved to exam room, continued coughing BP: 150/84 10:33 am 10:30am 50mg Benedryl 50mg given IM R deltoid, within 15 minutes coughing decreased and then stopped Emergency services had already been called. pt advised even though s/s decreased still needs ER evaluation as When Benedryl wears off COVID vaccine is still in his system. Pt agreeable and EMS transported pt to hospital VIA ambulance. Pt alert and oriented. L/n aware and present. Benedryl 50mg/ml Lot #080014 x 812022 1ML administered IM R deltoid
