Narrative: a71yo male with PMH of Tobacco Use, DJD, Vitamin D deficiency, Cervical Spondylosis without Myelopathy, GERD. Cough, Rectal Bleeding, Low Back Pain, General Weakness, Hypertension, Hyperlipidemia, Asymptomatic Carotid Artery Stenosis, Chronic gingivitis, BPH, Osteoarthritis, Pain of LT Shoulder Joint and ED. Per vaccine administering staff RN documentation; He received Pneumovax 0.5ml IM in left shoulder on September 16, 2021 and 10 to 15 minutes post vaccination, had itching at the injection site. The physician in charge was notified, oral Benadryl 25mg was ordered, administered and was tolerated well
