Symptomtext

Back upper left arm has been red, swollen and very painful in area that received the Varicella vaccine. Went to clinic and seen by NP on 4-19-22. States vaccine wasn't given deep enough and prescribed Cephalexin 500mg bid x 10 days. States no difference noted and states area burns. Patient also received Hepatitis B pediatric dose in error. Received 0.5 ml in Right Deltoid and received 0.5 ml in Left Deltoid when nurse realized had only given half of the required dose due to not administering an adult dose. Complaint of pain and swelling was not noted in either arm in the area that Hepatitis B vaccine was administered.

Symptomtext

Approximately 5 days after the vaccine was administered, the patient developed a red, circular area on the left thigh. At the time of her well child check up on 10/13/21, the varicella vaccine was given in the left leg and the MMR and Hep A vaccines were given in right leg. It may be an inflammatory reaction secondary to injection technique (suspicion that this vaccine may have been given IM instead of subcutaneous and that this may be the cause of this reaction). She presented for evaluation of this concern on 10/26/21. with her mother. At that time, I had a low concern for cellulitis or abscess since she was afebrile and over the past several days, the area of redness/concern had not increased in size. We marked area on the left leg with a surgical pen to watch for spreading of the erythema. Advised that the family can use warm compresses to the area. Also advised that they can use Zyrtec daily for itching.

Symptomtext

Per patient's mother patient was noted with swelling/redness to face primarily the cheeks, skin hot to the touch and chills about 8PM 10/18/2021. Around 1:00AM 10/19/2021 they called EMS and he was taken to the hospital. On the way to the hospital he vomited once in the ambulance. Patient's temperature was 103.4 at the hospital and was given Tylenol and Motrin. He was discharged after evaluation. Mother unaware of any other medications/treatment/diagnosis given at the hospital.

On an unknown date, the patient was vaccinated with measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (rHA) (M-M-R II) (lot # U006485, has been verified to be a valid lot number for measles, mumps, and rubella [wistar ra 27-3] virus vaccine, live [rHA] [M-M-R II], expiration date reported and upon internal validation established as 26-FEB-2023) (dose, anatomical location, route of administration, lot#, and expiration date were not reported) and varicella virus vaccine live (oka/merck) (VARIVAX) (lot # U002400, has been verified to be a valid lot number for varicella virus vaccine live [oka/merck] [VARIVAX], expiration date reported and upon internal validation established as 22-JAN-2023) (dose, anatomical location, route of administration, lot#, and expiration date were not reported).
