3/17/2022- immediate local reaction of erythema, hives, and pruritis that resolved w/hydrocortisone application and administration of diphenhydramine. Remained in office for 15 minutes after identification of reaction throughout time of treatment 3/23/2022- mom called office, patient c/o itching to left thigh w/large "rash" to area

No additional adverse events reported; a 12 month old patient received an extra dose of PROQUAD; This spontaneous report was received from a medical assistant and refers to a 12 month old female patient. No information regarding her medical history, drug reactions/allergies or concomitant therapies were was provided. On 02-AUG-2021, the patient was vaccinated with one dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA) (PROQUAD) 1 dose, lot # U002538, expiration date 24-JUL-2022 (exact dose and route of administration were not provided), for prophylaxis. On 16-AUG-2021, the patient was vaccinated with an extra dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA) (PROQUAD), 1 dose, lot # T035409, expiration date 14-APR-2022 (exact dose and route of administration were not provided), for prophylaxis. The patient did not experience any adverse event. She did not have any fever.

No additional adverse events reported; PROQUAD was inadvertently administered to an 18 year old patient; This spontaneous report was received from a certified medical assistant and refers to an 18-year-old patient of unknown gender. No information regarding the patient's pertinent medical history, drug reactions or allergies and concomitant therapies was provided. On 13-AUG-2021, the patient was inadvertently vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA)(PROQUAD) injection, lot # U002538, expiration date: 24-JUL-2022, 1 dose (injection site and route of administration were not provided) for prophylaxis (Product administered to patient of inappropriate age). The reporter stated that the patient was due for measles, mumps and rubella (reported as MMR) and varicella so she thought it would be easier to give one dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA)(PROQUAD) not thinking about the labeled age range. No additional adverse events were reported.

On 12-AUG-2021, the patient was vaccinated with an improperly stored dose of measles, mumps, rubella and varicella (Oka-Merck) Virus Vaccine Live albumin status rHA (recombinant Human albumin) (PROQUAD) 0.5 ml, once, lot# U002538, expiration date 24-JUL-2022, for prophylaxis (strength and route were not reported). No additional adverse event reported. The temperature excursion was of 5.7 Fahrenheit degrees (?F) during 10 minutes. A previous temperature excursion was not reported.
