Symptomtext

After vaccine was given, patient almost immediately developed an itchy, red rash on the back of both legs. Rash began to fade after about 3 minutes and fully resolved. He was given a dose of Benadryl and monitored; no subsequent reaction noted.

Sterile Injectable;N/A;Broken Syringe; No advese event; New PQC created for VAQTA syringe returned with lot U004366. Product arrived "broken" at the flange (please see attachments in case 02254862 for pictures of non labeled syringe). No additional PQC details or PQC. FROM CASE 02254862. THIS SYRINGE WAS RETURNED WITH ANOTHER BROKEN SYRINGE. CUSTOMER WILL BE CONTACTED FOR ANY ADDITIONAL INFORMATION. QUESTION SECTION: Medium Risk: Broken Syringe Barrel High Risk: Cracked or Broken Luer Lock (Needle Attachment Issues), Broken Finger Flange. Notes for DPOC: o A cracked syringe has a line or chip in the glass, but the syringe is essentially intact in one piece. A syringe is considered broken when the syringe is in 2 or more pieces. o A syringe is also considered "cracked" if it appears to be intact with evidence of a chip or crack OR appears to be intact initially but then breaks into 2 or more pieces during use (such as while attaching a needle or injecting diluent). o Please provide a detailed description of the defect using the questions below: o Is there a crack? On 04-OCT-2022, the patient was vaccinated with hepatitis a vaccine, inactivated (VAQTA) 0.5 milligram, intramuscular for prophylaxis, lot number U004366 has been verified to be valid for hepatitis a vaccine, inactivated expired date 01-OCT-2022 (expired product administered). HCP reports no further AE's; A 12 year old inadvertently received a 3rd dose of Hepatitis A vaccine; This spontaneous report has been received from a medical assistant, regarding a 12-year-old patient of unknown gender. Information regarding the patient's pertinent medical history, concomitant medications, concurrent conditions and previous drug reactions or allergies was unknown. On 22-MAR-2011, the patient was vaccinated with the first one dose of hepatitis A vaccine (manufacturer unknown) and on 16-FEB-2012 with the second dose of hepatitis A vaccine (manufacturer unknown) (formulation, strength, exact dose, vaccination scheme, route of administration and anatomical location, indication, lot number and expiration date were not provided). Moreover, on 17-MAR-2022, the patient was vaccinated inadvertently with a third dose of hepatitis A vaccine (VAQTA) syringe, at a dose of 0.5 milliliters (mL) one time as prophylaxis (lot number U004366 was verified as valid, expiration date reported and established as 01-OCT-2022) (route of administration and anatomical location date were not provided) (extra dose administered); on that same day the patient was also vaccinated with human papilloma virus (HPV) vaccine (manufacturer unknown) and with meningococcal vaccine a/c/y/w conj (dip tox) (MENACTRA). On 14-JAN-2022, the patient was vaccinated with a dose of hepatitis a vaccine, inactivated (VAQTA) pre-filled syringe intramuscular injection, 0.5 milliliter, lot #U004366, expiration date: 01-OCT-2022, administered for prophylaxis (strength, and route of administration were not provided) after a potential temperature excursion occurred. The data logger stopped recording for 24 hours, however, there were no specific temperatures to evaluate. The vaccine did not undergo previous temperature excursion. The patient did not experience any adverse event.

On 16-NOV-2021, the patient was vaccinated with improperly stored dose of hepatitis A vaccine, inactivated (VAQTA) lot # U004366, expiration date 01-OCT-2022 (dose, route of administration and anatomical location were not reported) for prophylaxis. Administered dose of hepatitis A vaccine, inactivated (VAQTA) experienced temperature excursion of 0.7 degrees Celsius for 2 hours. There was no previous excursion reported.
