7/1/22 at (0800) Patient emailed today to report he had a reaction to his MMR vaccine given in clinic earlier this week. I left a message for him to call back to provide more information as to what symptoms he was having with his reaction? Directed patient to contact company to make an appointment to be assessed. Notified company via email to let them know patient may call to schedule a visit today. 7/1/22 at (1445) Patient stopped by clinic, asked if he received my emails and phone message. He reports he did not receive any messages today. His right Lateral upper arm appeared pink in color w/ an approximate 2.5-3 inch rash like area. He reports he went to a local urgent care yesterday on 6/30/22. He was directed to take ibuprofen for the discomfort, but no further treatment was ordered. He did not appear short of breath or in any distress. I asked the pt to please contact company as previously instructed this morning per messages as there is a provider available if need to be further assessed. He agreed to call and see them today.
