Crying Immediate post-injection reaction Petechiae Rash macular Vomiting

Symptomtext

Client was sitting on mother's lap for vaccination. Interpreter present as family speaks another language. Site was cleansed and vaccinations were administered. Client immediately began to cry following vaccination. A few seconds after client began crying and getting worked up, client began having multiple episodes of emesis. Mom told interpreter that client was "making herself do it." Client had approximately 4-5 episodes of emesis. Emesis was off white in color. Mom reports that client ate right before vaccination appointment. Once client was done vomiting, client was observed in clinic room with RN for 15 minutes following vaccination. Client did have very few petechiae under both eyes following emesis. Mom reports client's face was blotchy, but once client calmed down and stopped crying the blotchiness resolved. Mom was educated on signs and symptoms to look out for and when to seek care prior to leaving clinic appointment. On 10/18/2022 called client's mom with interpreter to see how client was doing. Mom reports that client was doing well and did not even have a fever after vaccination. Recommend client not eat immediately prior to next vaccination appointment. Also explained that client should not have a completely empty stomach. Mom verbalized understanding and did not have any further questions.

No additional AE; HCP reporting an inadvertently administered MMR-II that was expired.; This spontaneous report has been received from a nurse, regarding to a patient of unknown gender and age. The patient's pertinent medical history, concurrent conditions, concomitant medications, and previous drug reactions or allergies were not reported. On 10-MAR-2023, the patient was vaccinated with an expired dose of measles, mumps, and rubella (Wistar RA 27-3) virus vaccine, live (rHA) (M-M-R II) injection, (lot number U007684 which has been verified to be valid, expiration date reported and validated and expiration date as 09-MAR-2023) (dose, strength, route of administration, and anatomical sites of injection were unknown by the reporter). in combination with sterile diluent (dose, strength, route of administration, anatomical sites of injection, lot number and expiration date were unknown by the reporter). No additional side effects were reported in the patient.

Symptomtext

Pediatrician ordered Dtap, PCV13, HepA, MMR, and Varicella vaccines for child visiting the clinic. During preparation the wrong vaccine was taken out, prepared, and administered. Instead of Dtap by itself, Pedarix (Dtap, HepB, IPV combo) was given in it's place by this RN. Child had NO adverse reaction, and had already received Hep B and IPV vaccines on a prior date. Pediatrician was notified within 10 minutes of administration mistake and mom was called immediately but was not able to talk to her until the next morning. Mom was notified of the incorrect vaccine given and mom reported child had no adverse reactions overnight and child slept well, drank, and ate as usual.

No adverse event; Nurse reporting improperly stored MMR II and VARIVAX in freezer; This spontaneous report was received from a nurse, referring to a 4-year-old child. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On an unknown date, it was reported that a nurse improperly stored measles, mumps, and rubella (WISTAR RA 27-3) virus vaccine, live (M-M-R II) and varicella virus vaccine live (OKA/MERCK) (VARIVAX) in the freezer (Product storage error). On 19-APR-2022 at 13:30, the patient received the first dose of measles, mumps, and rubella (WISTAR RA 27-3) virus vaccine, live (albumin status of rHA (recombinant human albumin)) (M-M-R II), valid lot #U007684 with expiration date on 09-MAR-2023 (strength, and route of administration not provided); and varicella virus vaccine live (OKA/MERCK) (VARIVAX), valid lot #U028164, with expiration date on 09-SEP-2023 (strength, and route of administration not provided). Both vaccines were administered as prophylaxis. No adverse events were reported. No additional information was provided. The causal assessment between the vaccines and the event were not reported.
