5/25/22 at 3:30am first witnessed seizure, fever 106 -> parents took private vehicle to Emergency room. 4:00am patient received Tylenol in ER and still lethargic and discharged home. 5:00am patient home. Motrin given 7:00am, 7:15am second seizure witnessed. 7:45 ambulance arrived and transported patient back to the emergency room. Patient had another seizure in route to the hospital witnessed by parent. Patient had fever 104.6 in ER and lethargic. Patient sent by private vehicle to hospital for an EEG. Then patient returned back and was observed overnight at ER. 5/26/2022 a rash appeared all over patients abdomen, face, partial down legs. 5/27/2022 another doctor appointment and no fever present. 5/30-31/2022 Rash started to resolve. Referred to Pediatric neurologist. Another EEG was preformed.

Symptomtext

Approximately 7 minutes after vaccine administration, client was standing and his eyes started to roll and started to fall to his left. Mom of client was able to catch client, before he hit anything on the ground. Client was unconscious for about 1 minute. 911 was called for EMS. When he came to, he was slightly disoriented. Clients color was pale and he was sweaty and complained of being hot. Client began to improve. Clients breathing was stable to the entire time. Client was laying in mothers lap and began to become disoriented again, and started to loose color in his face and sweat again. At this time Epi Autoinjector was given. EMS arrived approximately 2 minutes after Epi given. Clients breathing remained stable.

3/2/2022 (day of immunizations): fever onset at 18:30. Around 11:40 that night, 60-90 sec of convulsions, self-resolved, T102.4. Parents called EMS and infant was transported to local ED. Febrile on arrival (T101.5) to ED. No further convulsions and normal neurological exam.

Symptomtext

Large red swollen patch at sight of injections, L arm measuring at, R arm measuring at. Pt reported to medical and was treated with single dose of Benadryl (8 Mar 2023 @ 1200) and icing of the area. Skin marker was applied at the boarder of swelling/redness to keep track of spread.

On an unknown date, the patient received measles, mumps, and rubella (Wistar RA 27-3) virus vaccine, live (albumin status: recombinant human albumin [rHA]) (M-M-R II) 0.5 mL, (lot# U007685, expiration date: 09-MAR-2023) (strength, dose, route, and anatomical location were not provided) and measles, mumps, rubella and varicella (Oka-Merck) virus vaccine live (albumin status: rHA) (PROQUAD) 0.5 mL (lot# U028181, expiration date: 08-MAR-2023) (strength, dose, route, and anatomical location were not provided) both of them for prophylaxis, instead of measles, mumps, and rubella (Wistar RA 27-3) virus vaccine, live (M-M-R II) and varicella virus vaccine live (Oka/Merck) (VARIVAX) (Wrong product administered).
