Menveo given first, followed by Boostrix in the LD. HPV was then entered in the RD. Shortly after dose was administered, patient passed out for about 5-10 seconds and she jerked 2-3 times. Patient then came to, set up and then passed out again (5-10) seconds. Once patient awakened, she turned ashen in color & got cold and clammy. Kept patient laying down on table & took VS: B/P-82/44 R-brachial, T-97.4, P-54. Cold rag was applied to forehead. Asked patient if she had eating anything that morning & she stated that she had eaten an orange before coming to her Dr. appt. Had patient eat a granola bar and gave her some apple juice. Took B/P again after 10 minutes & it was up to 92/58 R-brachial. Patient no longer cold/clammy & her color had returned. Kept patient (along with her mother) in the office for at least an hour. She was feeling better after eating the granola bar & drinking the orange juice. Mother stated to this nurse that she thinks it was due to her daughter having some anxiety about getting 3 shots.

Patient rested on stretcher with legs extended. Ice pack applied to back of neck. Cool cloth applied to forehead. Patient sat up and dangled legs. BP 135/74, pulse 73 at 1345. States feeling better. Drank some water. 1410 - Patient up and moving around room. States feeling OK. No further complaints at present.

I (the pharmacist) gave the patient an HPV vaccine, then about 1 minute later gave a Pfizer vaccine. As I pulled out after administering the Pfizer vaccine, the patient began to lean back into the chair in a seemingly involuntary motion, and began shaking. I immediately thought this may have been a seizure, and grabbed behind her head to prevent it from banging on the table behind her. She seized for a few more seconds, then broke out of it, and fully regained consciousness. She described it as blacking out, and having lightheadedness and blurry vision afterwards. No Hx of seizures, but a few incidents of fainting spells have happened before when her blood sugar was low and she was anxious. She had skipped breakfast that morning, and was anxious about the shots (I couldn't tell just by look, but she said such afterwards), so perhaps a similar incident to the previous few, but with more visible shaking (vasovagal syncope perhaps?). Considering the timing and patient Hx, so quickly after 1 vaccine and during administration of another, and with possible precipitating factors, I doubt the vaccines were truly the cause, and the patient's stepmother agreed that they likely were not the cause. The patient was monitored for a half hour at the pharmacy, given some orange juice for possible low blood sugar, and then allowed to go home and rest, and call their PCP about the event. 911 was not called.

Syncopal episode after HPV vaccination today. Patient became Nauseous and fainted while sitting on exam chair. Pt laid down on exam table and cool compresses applied. Pt not shaking or convulsing. No dyspnea or Tachycardia. + diaphoresis. Pt. pale appearing - no cyanosis. BP taken several minutes after episode while pt diaphoretic and supine. Pt's color quickly returned and disposition returned to normal within 10-15 minutes within 10-15 minutes of laying down and drinking water, eating a snack. Pt did not have anything to eat today far ( episode happened about 12:00pm. Pt stating he was fine to ambulate out-parent requesting a wheelchair. Pt smiling ,alert, and conversant.

On the same date,was experiencing swelling at the injection site. The patient sought medical attention; however, no treatment was given for the adverse event. She developed a rash on the evening of 5/11/2022; This spontaneous report as received from a nurse practicioner (also reported as nurse prescriber) and refers to a 24-year-old female patient. Concomitant medications included oral contraceptive pills. No information regarding the patient's concurrent conditions and medical history was provided. On 11-MAY-2022, the patient was vaccinated intramuscularly with the first dose of HPV rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) injection, lot # U008316, expiration date repored as 14-AUG-2023, but upon internal validation established as 15-AUG-2023 (dose and anatomical location were not reported), for prevention of human papilloma virus infection. On the evening of the same day, the patient developed a rash, requiring the patient to go to the emergency department where she received intravenous diphenhydramine. No additional AE; administration of improperly stored vaccine; This spontaneous report was received from a registered nurse referring to a 10 years old female patient. There were no concomitant medications, concurrent conditions, medical history or drug reactions/allergies reported. There were no other vaccines within 4 weeks prior to the vaccine administration. On 18-MAR-2022, the patient was vaccinated with the first dose of HPV rL1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) which was improperly stored, 0.5 milliliter once in the right deltoid (strength and route not reported, lot # u008316, expiration date: 15-AUG-2023) for prophylaxis (product storage error). There was no illness at time of vaccination. The operator was fully trained, followed all direction and administration was cleared. The temperature excursion occurred when there was a unit malfunction, and the refrigerator worked out of range for 20 minutes or less before the vaccine was moved to a different unit that was in range. All vaccines were cleared as viable, no issues, no harm reached the patient. All vaccines were cleared for use. No measures need to be taken. The refrigerator was replaced. No additional adverse event was reported.

No further AE's were reported; when GARD ASIL 9 was administered to a patient some of the vaccine leaked down the patients arm; Exposure via skin contact; it was a user error with administering the vaccine; This spontaneous report was received from a pharmacist and refers to a patient of unknown age and gender. The patient's concomitant medications, concurrent conditions, medical history and drug allergies were not reported. On 23-MAR-2022, the patient was vaccinated with HPV rl1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) injection, at a dose of 0.5 ml one time, lot # U008316 was found to be valid, expiration date 15-AUG-2023 (route of administration and injection site were not reported) for prophylaxis. While vaccination, due to error with administering the vaccine (product administration error) some of the vaccine leaked down the patient's arm (accidental underdose, exposure via skin contact). No further adverse events (AE's) were reported.
