Patient complained of feeling dizzy. He fainted and looked pale. Student laid down flat with both feet elevated. O2 given, vital signs taken. No respiratory distress noted. Patient arousable. Alert and oriented. Denied feeling dizzy and able to get up on his own. Patient recovered well. No injury noted. Student went home with his mother.

Symptomtext

Pt. states she experienced right arm soreness right after receiving the vaccine. Pain worsened and spread down her arm. Tuesday (3/22/22) patient was seen in office for arm pain, no rash noted at that time. That evening she began with a bruise-like rash around the injection site. It has since spread and pain has worsened. Patient also describes paresthesias when using hand. Patient evaluated in office 3/24/22 for arm pain and rash - purpuric rash seen, sent to emergency department.

Pt complained of his right arm hurting about 3 1/2-4 hours after vaccine administration. He developed urticarial erythematous hives on his arm & back the following morning at 8am. No lip/throat swelling or itching. No shortness of breath or difficulty breathing. Seen on 7/14/22 in office w/ negative allergy testing & resolution of rash.

Symptomtext

Client came into the clinic for vaccinations on 6/8/22 with his guardian. This nurse reviewed the available record to determine what vaccines the child was to receive, per protocol. According to the only records available, after reviewing records multiple times, it was determined that that child needed Tdap, IPV, MMR, Varicella, Hep A, MenB, flu, and HPV. The client tolerated well the injections with no complications. Once the client left the office, this nurse had a conversation with the clinic support supervisor and the immunization supervisor and it was communicated at that time that the child had multiple records prior to this appointment. The clinic support supervisor had called records, rather than fill out a petition change form, to have the records merged and the result was an incorrect merge of the duplicate records. When the records were merged there were many vaccines that did not transfer over. Therefore, the "updated" record showed that he was incomplete with Tdap, IPV, MMR, Varicella, Hep A, MenB, flu, and HPV. With no documentation that this merge had occurred in the chart, the only records this nurse had to evaluate to determine what the child was due for was the "updated" record and the chart. Because of this, the child received Tdap, IPV, MMR, Varicella, Hep A, MenB, flu, and HPV. Now that the records have been corrected, it shows that the child received additional doses of MMR, Varicella, MenB, flu, and HPV that he did not need. Child's guardian was notified. No adverse events were reported. Child tolerated well.

The temperature and the time frame for the excursion was not provided. It was unknown if there was a previous excursion. No additional adverse events was provided (no adverse event). Patient was administered his 11 year vaccines (tdap, mcv4, hpv) on 5/12/2021 and had an appointment yesterday for his second hpv. Medical assistant says she checked and thought patient did not have his 11 year vaccines yet. Patient was administered his 11 year vaccines again on 3/15/2022 (tdap, mcv, hpv#2 and flu).
