Symptomtext

Awoke with an itchy, raised, little red dots rash all over his body. Pain upper right leg where vaccine was administered. Mother reports she administered Tylenol and Benadryl, and child has been sleeping ever since. States he acts normal otherwise, but plans to take him to Dr if she can get him an appointment.

The patient's pertinent medical history, drug allergies or reactions, and concomitant medications were unknown by the reporter. On 04-NOV-2022, the patient was vaccinated with an expired dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) subcutaneous injection, at a dose of 0.5 milliliter (1 dose) (route of administration, vaccination scheme frequency, and anatomical location were not provided; valid lot number U009259, with an expiration date of 19-SEP-2022) administered for prophylaxis (expired vaccine used). No additional AE information provided; Customer states that a partial dose of expired PROQUAD vaccine (LOT U009259 exp. 9/19/2022) was accidentally administered to a patient today, 9/21/2022.; This spontaneous report has been received from a nurse concerning a patient of unknown age and gender. The patient's concurrent conditions, medical history, concomitant therapies and drug/allergy history were not reported. On 21-SEP-2022 (also reported as "today"), the patient was accidentally administered an expired dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live(rHA) (PROQUAD) partial dose of 0.5 milliliter (ml) (lot # U009259, has been verified to be a valid lot number for measles, mumps, rubella and varicella [oka-merck] virus vaccine live[rHA] [PROQUAD], expiration date reported and upon internal validation established as 19-SEP-2022) (anatomical location and route of administration were not reported) with sterile diluent for prophylaxis (Expired product administered). The vaccine was properly stored with no temperature excursions. No additional adverse event was reported.

Patient received ProQuad and Varicella at this visit; This case was retrieved during the Vaccines Adverse Event Reporting System (VAERS) monitoring on 12-JAN-2022 with the following reference number: 1745229-1. The original narrative from the sender is as following: "Patient received ProQuad and Varicella at this visit." The causal relationship between Vaxelis and the event "Patient received ProQuad and Varicella at this visit" was assessed as not applicable due to the nature of the event. Of note, the event do not concern Vaxelis, but rather ProQuad and Varicella vaccines. Vaxelis should had been considered as concomitant. However, the case was databased to mirror what was reported in VAERS. No further information is expected

Information regarding the patient's concomitant therapies, concurrent conditions, medical history and historical drugs was not provided. On 25-AUG-2021, the patient was vaccinated with improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (dosage regimen was not reported), lot # U009259 (rHA) and expiration date 19-SEP-2022 administered as prophylaxis. The improperly stored dose of the suspect vaccine experienced a temperature excursion of -13.5 degree celsius (?C) for 6 hours and 39 minutes on an unknown date; no previous temperature excursions were reported.
